【摘要】：目的通過檢測穩(wěn)定性心絞痛、不穩(wěn)定性心絞痛和急性心肌梗死患者外周血NLRP3及相關(guān)因子IL-1β、IL-18的水平差異,分析NLRP3及相關(guān)因子IL-1β、IL-18與C反應(yīng)蛋白(CRP)、白細(xì)胞計(jì)數(shù)、左室射血分?jǐn)?shù)(LVEF)的相關(guān)性,初步了解NLRP3及相關(guān)因子在冠心病患者中的表達(dá)水平,探討NLRP3及相關(guān)因子與冠心病患者冠狀動脈粥樣斑塊不穩(wěn)定性的關(guān)系。方法選擇于2015年3月到2015年8月在廣西醫(yī)科大學(xué)第四附屬醫(yī)院(柳州市工人醫(yī)院)因胸痛住院的患者80例,根據(jù)冠心病診斷標(biāo)準(zhǔn)分型,其中急性心肌梗死(AMI組)患者26例,不穩(wěn)定性心絞痛(UAP組)患者28例,穩(wěn)定性心絞痛(SAP組)患者26例,所有患者均行冠狀動脈造影(CAG)檢查并明確診斷為冠心病,同期CAG正常的患者24例作為對照組(NC組)。用酶聯(lián)免疫吸附法(ELISA)測定每組患者外周血中NLRP3、IL-1β、IL-18蛋白的濃度,并應(yīng)用Pearson相關(guān)來分析NLRP3、IL-1β、IL-18和CRP、白細(xì)胞計(jì)數(shù)、LVEF之間的相關(guān)性。結(jié)果冠心病患者NLRP3、IL-1β外周血表達(dá)水平明顯高于對照組,AMI組的NLRP3、IL-1β及IL-18蛋白水平均顯著高于UAP組、SAP組和NC組(均P0.05),UAP組和SAP組的NLRP3及IL-1β蛋白水平明顯高于NC組(均P0.05),UAP組的NLRP3蛋白水平明顯高于SAP組(F=5.071,P=0.029),且冠心病患者血漿NLRP3、IL-1β及IL-18蛋白水平分別與白細(xì)胞計(jì)數(shù)及CRP呈正相關(guān)關(guān)系,與LVEF為負(fù)相關(guān)的關(guān)系。結(jié)論在冠心病患者中,AMI組及UAP組患者的外周血NLRP3蛋白濃度明顯高于SAP組,提示NLRP3的高表達(dá)可能與冠心病患者冠狀動脈粥樣斑塊的不穩(wěn)定性相關(guān),且冠心病患者外周血NLRP3、IL-1β及IL-18蛋白水平與LVEF均為負(fù)相關(guān)關(guān)系。
[Abstract]:Objective to detect the levels of NLRP3 and IL-1 尾 -IL-18 in peripheral blood of patients with stable angina pectoris, unstable angina pectoris and acute myocardial infarction. The correlation of left ventricular ejection fraction (LVEF) and the expression of NLRP3 and related factors in patients with coronary heart disease. To investigate the relationship between NLRP3 and related factors and the instability of coronary atherosclerotic plaque in coronary heart disease patients, 80 patients with chest pain were selected from March 2015 to August 2015 in the fourth affiliated Hospital of Guangxi Medical University (Liuzhou Workers Hospital). According to the diagnostic criteria of coronary heart disease, 26 patients with acute myocardial infarction (AMI), 28 patients with unstable angina pectoris (UAP), 26 patients with stable angina pectoris (SAP) and 26 patients with stable angina pectoris (SAP) were classified. All patients were examined by coronary angiography (CAG) and clearly diagnosed as coronary heart disease (CHD). 24 patients with normal CAG were used as control group. Elisa was used to detect the level of IL-1 尾 -IL-18 protein in peripheral blood of each group. Pearson correlation was used to analyze the correlation between IL-18, CRP, WBC count and IL-18. Results the levels of IL-1 尾 and IL-18 in peripheral blood of patients with coronary heart disease were significantly higher than those of control group (P < 0.05). The levels of NLRP3 and IL-1 尾 protein in both P0.05 and SAP groups were significantly higher than those in NC group (both P0.05 / UAP group and SAP group were significantly higher than that in SAP group. There was a positive correlation between the plasma levels of NLRP3P IL-1 尾 and IL-18 protein and leukocyte count and CRP in patients with coronary heart disease. Conclusion the concentration of NLRP3 protein in peripheral blood of patients with coronary heart disease is significantly higher than that of SAP group, suggesting that the high expression of NLRP3 may be related to the instability of coronary atherosclerotic plaque in patients with coronary heart disease. The levels of IL 1 尾 and IL-18 protein in peripheral blood of patients with coronary heart disease were negatively correlated with LVEF.
