不同類型冠心病患者Lp-PLA2基因N133K位點(diǎn)多態(tài)性的意義

【摘要】:目的檢測(cè)人脂蛋白相關(guān)磷脂酶A2(Lp-PLA2)基因N133K位點(diǎn)多態(tài)性與不同類型冠心病之間的相關(guān)性及臨床意義。方法以心絞痛患者426例為心絞痛組、心肌梗死患者415例為心梗組,以同期體檢健康者400例為對(duì)照組。應(yīng)用聚合酶鏈反應(yīng)(PCR)分析技術(shù)結(jié)合DNA直接測(cè)序技術(shù),對(duì)入選者Lp-PLA2基因N133K(C/G)位點(diǎn)多態(tài)性進(jìn)行分型。采用多因素logistic回歸分析Lp-PLA2基因N133K多態(tài)位點(diǎn)與不同類型冠心病的相關(guān)性。同時(shí)檢測(cè)入組者血脂及Lp-PLA2水平等指標(biāo)。結(jié)果總膽固醇(TC)、三酰甘油(TG)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDL-C)、Lp-PLA2水平在3組間差異有統(tǒng)計(jì)學(xué)意義(均P0.05),其中心梗組TC、TG、LDL-C、Lp-PLA2高于心絞痛組和對(duì)照組,HDL-C低于心絞痛組和對(duì)照組,心絞痛組TC、TG、LDL-C、Lp-PLA2高于對(duì)照組,HDL-C低于對(duì)照組(均P0.05)。心梗組和心絞痛組1個(gè)月后復(fù)查時(shí),Lp-PLA2水平均明顯降低(均P0.05)。心梗組GG、GC基因型以及G等位基因的分布頻率明顯高于心絞痛組和對(duì)照組,心絞痛組GG、GC基因型以及G等位基因的分布頻率明顯高于對(duì)照組(均P0.05)。Logistic回歸分析發(fā)現(xiàn),Lp-PLA2基因N133K(C/G)多態(tài)位點(diǎn)GG基因型與冠心病的發(fā)生有顯著的相關(guān)性(P0.01)。結(jié)論Lp-PLA2基因N133K(C/G)多態(tài)位點(diǎn)基因突變可能與冠心病病情有關(guān)。
[Abstract]:Objective to investigate the relationship between N133K locus polymorphism of human lipoprotein associated phospholipase A _ 2 (Lp-PLA2) gene and different types of coronary heart disease (CHD). Methods 426 patients with angina pectoris and 415 patients with myocardial infarction were selected as patients with angina pectoris (angina pectoris) and 415 patients with myocardial infarction (MI). In the same period, 400 healthy persons were used as control group. Polymerase chain reaction (PCR) and DNA direct sequencing were used. The polymorphism of Lp-PLA2 gene N133K / C / G was genotyped. Multivariate logistic regression analysis was used to analyze the correlation between N133K polymorphism of Lp-PLA2 gene and different types of coronary heart disease. Blood lipid and Lp-PLA2 level were also detected. There were significant differences in the levels of Lp-PLA2 among the three groups (P < 0.05), among which the levels of LDL-CU Lp-PLA2 in the myocardial infarction group were higher than those in the angina pectoris group and the control group were lower than those in the angina pectoris group and the control group (P < 0.05), and the levels of Lp-PLA2 were significantly higher in the myocardial infarction group than in the angina pectoris group and the control group (P < 0.05), and the levels of Lp-PLA2 were significantly higher in the MI group than in the angina pectoris group and the control group. The level of Lp-PLA2 in angina group was significantly lower than that in control group (P 0.05). The level of Lp-PLA2 in MI group and angina pectoris group was significantly lower than that in angina group after 1 month (P 0.05). The frequency of GGG GC genotype and G allele in MI group was significantly higher than that in angina pectoris group and control group. The frequency of GC genotype and G allele in angina pectoris group was significantly higher than that in control group (all P0.05).Logistic regression analysis showed that there was a significant correlation between GG genotype of Lp-PLA2 gene N133KU C / G) and the occurrence of coronary heart disease. Conclusion the Lp-PLA2 gene N133KU CG / C / G is more than that of control group (P < 0.05). Conclusion there is a significant correlation between GG genotype of Lp-PLA2 gene and the occurrence of coronary heart disease. The mutation of the locus gene may be related to the condition of coronary heart disease.
