【摘要】：目的觀察米力農(nóng)聯(lián)合心通口服液治療慢性心力衰竭的臨床療效以及對患者生活質(zhì)量影響。方法將92例慢性充血性心力衰竭患者隨機分成兩組,其中聯(lián)合用藥組46人,對照組46人。所有患者均采取使用利尿劑、口服血管緊張素轉(zhuǎn)換酶抑制藥等常規(guī)治療,聯(lián)合用藥組患者在常規(guī)治療基礎(chǔ)上采用米力農(nóng)聯(lián)合心通口服液治療,對照組患者在常規(guī)治療基礎(chǔ)上單獨應(yīng)用米力農(nóng)治療。從臨床療效、血漿N末端B型利鈉肽(NT-proBNP),左心室舒張末期內(nèi)徑值及左心室射血分數(shù)指標進行評價,同時還監(jiān)測患者的肝功能和腎功能變化。結(jié)果 聯(lián)合用藥組臨床療效總有效率明顯的高于米力農(nóng)組,兩組比較差異有統(tǒng)計學意義(63%VS.82.6%,P0.05)。經(jīng)過兩個療程(6周)治療后與治療前比較,聯(lián)合用藥組和米力農(nóng)組左心室射血分數(shù)(LVEF)水平均明顯增加,差異具有統(tǒng)計學意義(P0.05);聯(lián)合用藥組較米力農(nóng)組LVEF升高更明顯,兩組間比較差異有統(tǒng)計學意義(P0.05)。經(jīng)過兩個療程(6周)治療后,聯(lián)合用藥組和米力農(nóng)組血漿N末端B型利鈉肽(NT-pro BNP)明顯下降,差異具有統(tǒng)計學意義(P0.05);聯(lián)合用藥組較米力農(nóng)組血漿NT-BNP下降更明顯,兩組間比較有統(tǒng)計學意義(P0.05)。聯(lián)合用藥組及米力農(nóng)組左心室舒張末期內(nèi)徑(LVD)治療前后未見明顯的變化。聯(lián)合用藥組及米力農(nóng)組肝功能、腎功能指標在治療前與治療后未見明顯的變化。結(jié)論米力農(nóng)注射液在加用心通口服液治療CHF的臨床效果明顯的好于單獨的應(yīng)用米力農(nóng)治療CHF。
[Abstract]:Objective to observe the clinical effect of milrinone combined with Xintong oral liquid on the quality of life of patients with chronic heart failure. Methods 92 patients with chronic congestive heart failure were randomly divided into two groups, including 46 patients in combination group. 46 patients in the control group were treated with diuretic, oral angiotensin converting enzyme inhibitor and other routine therapy. The patients in the combined treatment group were treated with milrinone combined with Xintong oral liquid on the basis of routine treatment. The control group was treated with milrinone alone on the basis of routine therapy. The clinical efficacy, plasma N-terminal B-type natriuretic peptide NT-proBNPN, left ventricular end-diastolic diameter and left ventricular ejection fraction (LVEF) were evaluated. Results the total effective rate of the combined treatment group was significantly higher than that of the milrinone group, and the difference between the two groups was statistically significant (P 0.05). After two courses of treatment (6 weeks), the total effective rate was compared with that before treatment. The level of left ventricular ejection fraction (LVEF) in combination group and milrinone group was significantly increased, the difference was statistically significant (P 0.05), and the level of LVEF in combination group was significantly higher than that in milrinone group. The difference between the two groups was statistically significant (P 0.05). After two courses of treatment (6 weeks), the plasma N-terminal natriuretic peptide N-terminal type B natriuretic peptide (NT-pro BNPs) in the combined treatment group and milrinone group decreased significantly. The difference was statistically significant (P 0.05), the plasma NT-BNP in combination group was significantly lower than that in milrinone group. There was no significant change in left ventricular end-diastolic diameter before and after treatment in combination group and milrinone group. Liver function was observed in combination group and milrinone group. Conclusion the clinical effect of milrinone injection in the treatment of CHF is better than that in the single application of milrinone in the treatment of CHF.
