實時三維超聲成像技術(shù)對頸動脈粥樣斑塊體積測定的定量分析

【摘要】:目的:探討實時三維超聲成像(RT-3DU)技術(shù)用于定量分析頸動脈粥樣硬化斑塊體積的可行性及準確性。方法:對23例頸動脈粥樣硬化患者行實時三維超聲檢查,對獲取的35個斑塊進行三維重建,所有斑塊體積由2位超聲醫(yī)師進行隨機雙盲獨立測定,1個月后,由其中1位醫(yī)師再次進行測定。結(jié)果:RT-3DU定量測量的35個斑塊的體積為(171±116)mm3。對于斑塊體積的定量測量,兩幅Bland-Altman圖中僅見2.9%(1/35)的分散點在一致性界限外,兩幅圖差值均數(shù)的絕對值與差值均數(shù)為0的基線最大僅相差1.51mm3;對于管腔體積狹窄率的定量測量,兩幅Bland-Altman圖中僅見2.9%(1/35)的分散點在一致性界限外,兩幅圖差值均數(shù)的絕對值與差值均數(shù)為0的基線最大僅相差0.63%;2位觀察者間及觀察者內(nèi)4組數(shù)據(jù)的組內(nèi)相關(guān)系數(shù)均≥0.900,表示觀察者間和觀察者內(nèi)部的一致性較高。結(jié)論:RT-3DU可以立體顯示斑塊的空間形態(tài),斑塊體積及管腔體積狹窄率的定量測量在觀察者間和觀察者內(nèi)部均有較高的一致性,為動脈粥樣硬化斑塊的定性診斷和定量測量提供了新的檢查手段。
[Abstract]:Objective: to investigate the feasibility and accuracy of real-time three-dimensional ultrasound (RT-3DU) technique for quantitative analysis of carotid atherosclerotic plaque volume. Methods: 23 patients with carotid atherosclerosis were examined by real-time three-dimensional ultrasound.
