【摘要】：目的探討彩色多普勒超聲心動圖(CDE)對老年房間隔缺損(ASD)患者右心室形態(tài)及收縮功能的評估價值。方法超聲科收治的60例老年ASD患者為ASD組,根據(jù)不同的肺動脈收縮壓(PASP)將ASD患者分為無肺動脈高壓患者(PASP35 mm Hg,低壓組)29例和肺動脈高壓患者(PASP35 mm Hg,高壓組)31例,并選取該院同期體檢正常的成人志愿者30例作為對照組。使用實時三維超聲心動圖(RT-3DE)對研究對象的右心室形態(tài)及收縮功能進行檢測和分析,分析參數(shù)包括:右心室舒張末期內(nèi)徑(RVDD)、右心室舒張末期容積(RVEDV)、右心室收縮末期容積(RVESV)、右心室每搏量(RVSV)、右心室射血分數(shù)(RVEF)。結(jié)果 ASD組RVDD、RVEDV、RVESV及RVSV均顯著高于對照組,而RVEF顯著低于對照組(P0.05)。高壓組RVDD、RVEDV、RVESV、RVSV顯著高于低壓組和對照組,RVEF顯著低于低壓組和對照組,高壓組ASD最大直徑顯著高于低壓組(P0.05),低壓組RVDD、RVEDV、RVESV、RVSV及RVEF與對照組比較無統(tǒng)計學(xué)差異(P0.05)。經(jīng)Pearson相關(guān)分析顯示,ASD患者RVEF與患者ASD最大直徑、RVDD、RVEDV、RVESV呈負相關(guān)(r=-0.387,-0.352,-0.226,-0.302;P=0.015,0.000,0.012,0.023),與RVSV無相關(guān)性(r=0.182,P=0.253)。結(jié)論 RT-3DE對老年ASD患者的右心室形態(tài)及收縮功能評估具有高準(zhǔn)確性。
[Abstract]:Objective to evaluate the value of color Doppler echocardiography (CDE) in evaluating right ventricular morphology and systolic function in elderly patients with atrial septal defect (ASD). According to different pulmonary arterial systolic pressure (PASP), the patients with ASD were divided into three groups: PASP35mm Hg without pulmonary hypertension, 29 patients with hypobaric hypertension, 35mm PG with pulmonary hypertension and 31 patients with hypertension. Thirty adult volunteers with normal physical examination were selected as control group. The right ventricular morphology and systolic function were measured and analyzed by real-time three-dimensional echocardiography (RT-3DE). The parameters included right ventricular end-diastolic diameter (RVDDD), right ventricular end-diastolic volume (RVEDVV), right ventricular end-systolic volume (RVESVV), right ventricular volume (RVSVV), right ventricular ejection fraction (RVEFV). Results RVDDV, RVEDVV, RVESV and RVSV in ASD group were significantly higher than those in control group. RVEF was significantly lower than that of control group (P 0.05). The maximum diameter of ASD in the high pressure group was significantly higher than that in the low pressure group (P 0.05). There was no significant difference in RV VSV and RVEF between the low pressure group and the control group. The Pearson correlation analysis showed that there was a negative correlation between RVEF and the maximum diameter of ASD. RT-3DE has high accuracy in evaluating right ventricular morphology and systolic function in elderly patients with ASD.
