【摘要】：目的:探討曲美他嗪聯(lián)合運(yùn)動(dòng)療法對(duì)冠心病經(jīng)皮冠狀動(dòng)脈介入(PCI)術(shù)后患者心臟康復(fù)作用的影響。方法:納入冠心病PCI術(shù)后1個(gè)月內(nèi)的患者90例,隨機(jī)分成治療A組、治療B組及對(duì)照組,每組各30例。A組在冠心病二級(jí)預(yù)防治療的基礎(chǔ)上加用曲美他嗪并聯(lián)合運(yùn)動(dòng)療法、B組在冠心病二級(jí)預(yù)防治療的基礎(chǔ)上加用曲美他嗪、對(duì)照組僅給予冠心病二級(jí)預(yù)防治療。觀察期為6個(gè)月。監(jiān)測(cè)3組患者的6 min步行試驗(yàn)(6MWT)的距離、6MWT時(shí)的心率及動(dòng)脈血氧飽和度、左室舒張末期內(nèi)徑(LVEDD)、左室收縮末期內(nèi)經(jīng)(LVESD)、左室間隔厚度(LVIVS)、左心室后壁厚度(LVPWT)、左室射血分?jǐn)?shù)(LVEF)及正常舒張功能的比例。結(jié)果:組內(nèi)比較:與治療前相比,3組的6MWT的距離明顯增多、6MWT時(shí)的心率明顯降低、正常舒張功能的比例明顯升高并且LVEF明顯增加,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。治療后組間比較:A組分別與B組及對(duì)照組相比,6MWT的距離明顯增加、6MWT時(shí)的心率明顯降低、正常舒張功能的比例明顯升高且左室射血分?jǐn)?shù)明顯增加,差異具有統(tǒng)計(jì)學(xué)意義(P0.017);B組與對(duì)照組相比,治療后LVEF明顯增加,差異具有統(tǒng)計(jì)學(xué)意義(P0.017),6MWT的距離、6MWT時(shí)的心率及正常舒張功能的比例兩組無(wú)明顯改善(P0.05)。結(jié)論:曲美他嗪聯(lián)合運(yùn)動(dòng)療法對(duì)冠心病PCI術(shù)后患者的心臟康復(fù)效果優(yōu)于常規(guī)的冠心病二級(jí)預(yù)防治療。
[Abstract]:Objective: to investigate the effect of trimetazidine combined with exercise therapy on cardiac rehabilitation in patients with coronary heart disease after percutaneous coronary intervention (PCI). In group B and control group, 30 cases in each group were treated with trimetazidine combined with exercise therapy on the basis of secondary prophylaxis of coronary heart disease, and trimetazidine was added in group B on the basis of secondary prophylaxis of coronary heart disease. The control group was only treated with secondary prophylaxis of coronary heart disease. The observation period was 6 months. The heart rate and arterial oxygen saturation were monitored during 6 min walking test (6 MWT) and 6 MWT. Left ventricular end-diastolic diameter (LVEDDN), left ventricular end-systolic dimension (LVESD), left ventricular septal thickness (LVIVSN), left ventricular posterior wall thickness (LVPWTT), left ventricular ejection fraction (LVEF) and normal diastolic function were compared. The heart rate decreased significantly when the distance increased significantly by 6MWT. The ratio of normal diastolic function and LVEF were significantly increased, the difference was statistically significant (P 0.05). After treatment, compared with group B and control group, the heart rate of group A was significantly lower than that of group B, and the distance between group A and group B was significantly higher than that of group B and control group (P < 0.05). The ratio of normal diastolic function and left ventricular ejection fraction were significantly increased. The difference was statistically significant. Compared with the control group, LVEF in group B was significantly higher than that in control group. There was no significant difference in heart rate and normal diastolic function between the two groups under 6MWT. Conclusion: the effect of combined exercise therapy with trimetazidine on cardiac rehabilitation after PCI is better than that of routine therapy. Secondary prophylaxis for coronary heart disease.
