血漿suPAR與急性冠脈綜合征的相關(guān)性以及其影響因素的研究

【摘要】:目的:探討血漿可溶性尿激酶型纖溶酶原激活物受體(su PAR)水平與急性冠脈綜合征(ACS)患者冠狀動(dòng)脈病變程度及其影響因素。方法:收集蘭州大學(xué)第一醫(yī)院心臟中心2015年3月-2016年9月因胸痛住院321例患者的基本資料,根據(jù)臨床表現(xiàn)及冠狀動(dòng)脈造影結(jié)果分為急性冠脈綜合征(ACS)組228例,冠狀動(dòng)脈造影正常的對(duì)照組93例,采用標(biāo)準(zhǔn)的酶聯(lián)免疫吸附法(ELISA法)檢測(cè)血漿su PAR、高敏C反應(yīng)蛋白(hs-CRP)水平并進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:血漿su PAR水平是ACS發(fā)生的危險(xiǎn)因素[OR=0.104,P0.01,95%(可信區(qū)間)CI:0.048-0.223],ACS組血漿su PAR水平高于對(duì)照組(P0.01),在ACS組中隨著Gensini積分增高,血漿su PAR水平逐漸升高;根據(jù)ROC曲線獲得su PAR預(yù)測(cè)ACS對(duì)應(yīng)界值:1.8ug/L,敏感度0.732,特異性0.71,多元線性回歸分析提示吸煙、Gensini積分、尿酸是影響血漿su PAR水平主要因素。結(jié)論:血漿su PAR水平的升高與ACS患者冠狀動(dòng)脈病變程度相關(guān),且隨著冠狀動(dòng)脈病變程度的Gensini積分升高,血漿中su PAR水平呈遞增趨勢(shì),這種關(guān)系優(yōu)于傳統(tǒng)炎性標(biāo)記物hs-CRP對(duì)ACS患者的預(yù)測(cè),有可能成為預(yù)測(cè)ACS患者冠狀動(dòng)脈狹窄程度及危險(xiǎn)分層的新的生物標(biāo)記物。
[Abstract]:Objective: to investigate the relationship between plasma soluble urokinase plasminogen activator receptor (plasminogen activator receptor) and the severity of coronary artery disease in patients with acute coronary syndrome (ACSS) and its influencing factors. Methods: the heart center of the first Hospital of Lanzhou University was collected. Basic data of 321 patients hospitalized with chest pain from March 2015 to September 2016, According to clinical manifestations and coronary angiography results, 228 patients were divided into acute coronary syndrome (ACS) group and 93 patients with normal coronary angiography (control group). The plasma levels of su par, Gao Min C reactive protein hs-CRP were measured by standard Elisa and analyzed statistically. Results: plasma su PAR level was a risk factor for the occurrence of ACS. The average level was higher than that in the control group (P 0.01), and increased with the increase of Gensini score in the ACS group. According to the ROC curve, su PAR predicted the corresponding threshold value of ACS: 1.8ugr / L, sensitivity 0.732, specificity 0.71. Multiple linear regression analysis showed that smoking had Gensini score. Conclusion: the increase of plasma su PAR level is related to the degree of coronary artery lesion in patients with ACS, and with the increase of Gensini score of coronary artery lesion, the su PAR level in plasma tends to increase. This relationship is superior to that of traditional inflammatory marker hs-CRP in predicting ACS patients and may be a new biomarker for predicting coronary artery stenosis and risk stratification in ACS patients.
