【摘要】：目的:目前冷凍球囊消融和射頻消融是用于房顫導管消融的兩種不同方法;然而,關于哪種消融方法更好并沒有統(tǒng)一共識。因此我們嘗試針對已發(fā)表的隨機對照臨床試驗進行薈萃分析比較冷凍消融與射頻消融的有效性和安全性,從而為臨床合理治療提供循證學依據(jù)。方法:在PubMed,EMBASE,CNKI和萬方數(shù)據(jù)庫檢索截止2016年5月發(fā)表的對比冷凍球囊消融和射頻消融用于房顫患者消融治療的隨機對照研究,篩選目的試驗進行相關文獻的質(zhì)量評價,提取所需要的實驗數(shù)據(jù),并使用RevMan軟件5.3版本對研究數(shù)據(jù)進行分析,比較兩種消融方式治療房顫的復發(fā)率、并發(fā)癥發(fā)生率、手術時間及曝光時間等相關數(shù)據(jù)并對這些數(shù)據(jù)進行薈萃分析。結果:最終納入6項研究,所有的臨床試驗都是在2012年至2016年之間發(fā)表的,納入的6項研究均為隨機對照試驗;共包括642名接受了冷凍球囊消融治療的患者,651名接受了射頻消融治療的患者。薈萃分析結果提示:和射頻消融相比,使用冷凍球囊消融的患者,其有效性(房顫復發(fā)率)無明顯差異(I2=0%,P=0.57);冷凍球囊消融組的并發(fā)癥多于射頻消融(I2=0%,P=0.005);但冷凍球囊消融組的手術總時長較射頻消融組顯著縮短(I2=31%,P0.05)。結論:和射頻消融相比,冷凍球囊消融其有效性無明顯差異,但冷凍球囊消融組的手術總時長較射頻消融組減少,然而冷凍球囊消融組的并發(fā)癥卻多于射頻消融,關于兩種消融方法的安全性和有效性的對比還需要一些大型的臨床研究進一步證實。
[Abstract]:Objective: cryoballoon ablation and radiofrequency ablation is currently used two different methods for catheter ablation of atrial fibrillation; however, about what kind of better ablation method and there is no uniform consensus. So we try to freeze a meta-analysis was performed to compare the efficacy and safety of ablation and radiofrequency ablation for randomized controlled clinical trials have been published, so as to provide evidence-based basis for clinical treatment. Methods: PubMed, EMBASE, CNKI and Wanfang database published by the end of May 2016 comparing cryoballoon ablation and radiofrequency ablation for patients with atrial fibrillation ablation with treatment of the quality evaluation to select the test of related literature, experimental data to be extracted, and 5.3 the version of the research data for analysis using RevMan software, comparison of two kinds of ablation treatment of atrial fibrillation recurrence rate, complication rate, operation time and exposure time. The relevant data and perform a meta-analysis of these data. Results: a total of 6 studies, all clinical trials are published between 2012 to 2016, 6 included studies were randomized controlled trials; including a total of 642 patients received cryoballoon ablation therapy, 651 patients received radiofrequency ablation therapy a meta-analysis of patients. Results: radiofrequency catheter ablation and compared using cryoballoon ablation patients, its effectiveness (the recurrence rate of atrial fibrillation) had no significant difference (I2=0%, P=0.57); cryoballoon ablation group had more complications than radiofrequency ablation (I2=0%, P=0.005); but the cryoballoon ablation group of surgery, the total length of a radio frequency the ablation group was significantly shortened (I2=31%, P0.05). Conclusion: radiofrequency catheter ablation and compared the effectiveness of cryoballoon ablation has no obvious difference, but the cryoballoon ablation group operation is the total length of the ablation group decreased, however cryoballoon ablation The complication of the group is more than the radiofrequency ablation. The comparison of the safety and effectiveness of the two ablation methods is further confirmed by a number of large clinical studies.
