Reg IV在炎癥性腸病的表達(dá)及其對(duì)病變活動(dòng)度的預(yù)測(cè)作用

背景和目的炎癥性腸。╥nflammatory bowel disease, IBD)是一組病因未明的非特異性腸道炎癥性疾病,包括潰瘍性結(jié)腸炎(ulcerative colitis, UC)和克羅恩。–rohn’s disease, CD)。近二十余年來(lái),隨著人們生活方式的變化和診斷水平的提高,國(guó)內(nèi)炎癥性腸病的發(fā)病率有逐年上升的趨勢(shì)。疾病是處于活動(dòng)期還是靜止期直接關(guān)系到疾病的正確治療。IBD臨床表現(xiàn)復(fù)雜多變,診斷缺乏特異性的指標(biāo),而且常因?qū)Σ∏榛顒?dòng)的判斷不準(zhǔn)確而延誤治療。目前臨床評(píng)價(jià)IBD活動(dòng)與否主要通過(guò)臨床癥狀、實(shí)驗(yàn)室生化指標(biāo)、內(nèi)鏡下的表現(xiàn)及病理等綜合判斷。其中C-反應(yīng)蛋白(CRP)幾乎全部由肝臟產(chǎn)生,短時(shí)間內(nèi)(數(shù)小時(shí))即可升高,而機(jī)體一旦恢復(fù),CRP又迅速恢復(fù)正常(6~7d)。CRP可及時(shí)而敏感地反映炎癥活動(dòng),是目前臨床最為推崇的IBD活動(dòng)性指標(biāo)之一。另外,各國(guó)專家針對(duì)IBD制定出疾病活動(dòng)指數(shù)(disease activity index),對(duì)疾病的活動(dòng)性進(jìn)行量化處理,從而更客觀準(zhǔn)確地反映疾病的活動(dòng)性,幫助臨床醫(yī)師作出正確的醫(yī)療決策。評(píng)價(jià)UC活動(dòng)度常采用Southerland疾病...
