安裝透明帽并二氧化碳?xì)怏w注入對結(jié)腸鏡檢查進(jìn)鏡速度及結(jié)腸息肉檢出率的影響

【摘要】:目的觀察并探討腸鏡前端安裝透明帽聯(lián)合二氧化碳CO2氣體注入對結(jié)腸鏡檢查進(jìn)鏡速度及鏡下息肉檢出率的影響。方法將2014年1月-2016年6月擬行結(jié)腸鏡檢查的216例患者據(jù)隨機數(shù)字表分為A、B、C 3組,每組72例,A組患者腸鏡前端安裝透明帽而后行結(jié)腸鏡檢查,B組檢查前腸鏡不安裝透明帽,進(jìn)鏡前腸管內(nèi)注入CO2氣體,C組聯(lián)合采用上述兩種方法而后再行腸鏡檢查,比較3組患者檢查過程中進(jìn)鏡速度及息肉檢出率差異。結(jié)果 A、B、C 3組受檢者腸鏡達(dá)到回盲部比例差異無統(tǒng)計學(xué)意義(95.8%vs 90.3%vs 98.6%)(χ2=5.34,P=0.069);到達(dá)回盲部平均時間C組A組B組[(7.14±1.12)vs(8.75±1.33)vs(9.03±1.57)min,F=3.75,P=0.025]。3組總體息肉檢出率C組A組B組(38.9%vs 34.7%vs 23.6%)(χ2=4.10,P=0.129),A、B、C 3組微小結(jié)腸息肉檢出率(30.6%vs 18.1%vs 36.1%)及右半結(jié)腸息肉檢出率(20.8%vs 8.3%vs 23.6%)差異均有統(tǒng)計學(xué)意義(χ2=6.08,P=0.048;χ2=6.58,P=0.037);颊叱风R后10 min[(4.35±0.62)vs(4.88±0.85)vs(5.57±1.05)]和30 min[(3.04±0.37)vs(3.30±0.56)vs(4.41±0.84)]VAS評分C組B組A組(F=4.85,P=0.009;F=3.26,P=0.040)。結(jié)論結(jié)腸鏡檢查時鏡端安置透明帽聯(lián)合CO2注入可顯著提高結(jié)腸鏡進(jìn)鏡速度與結(jié)腸息肉檢出率,對微小結(jié)腸息肉及右半結(jié)腸息肉的檢出更具優(yōu)勢。
