【摘要】：目的探討白細(xì)胞介素(IL)-12B及其受體在炎癥性腸病(IBD)患者外周血及腸黏膜中的表達和意義。方法通過結(jié)腸鏡下收取結(jié)腸黏膜標(biāo)本,其中活動期的克羅恩病(CD)患者、潰瘍性結(jié)腸炎(UC)患者各60例,健康對照者50例。使用Western blot對活動期IBD患者發(fā)病部位腸黏膜蛋白水平進行檢測,以同期結(jié)腸鏡檢查未見異常的患者作為對照。收集患者及健康體檢者的外周血2 mL,留取血漿,提取血細(xì)胞總RNA,用Realtime PCR法檢測IL-12B及其受體IL-12RB1的mRNA表達,用酶聯(lián)免疫吸附試驗(ELISA)法檢測血漿IL-12p40的表達。結(jié)果外周血Real-time PCR結(jié)果顯示IL-12B mRNA在CD、UC患者中的表達均增高,且差異有統(tǒng)計學(xué)意義(P0.05);IL-12RB1的mRNA在CD、UC患者中表達也增高,但只有CD患者有統(tǒng)計學(xué)意義(P0.05)。腸黏膜Real-time PCR結(jié)果顯示IL-12B mRNA在CD、UC患者中的表達均增高,且差異有統(tǒng)計學(xué)意義(P0.05);而IL-12RB1 mRNA的表達在3組之間差異無統(tǒng)計學(xué)意義(P0.05)。外周血及腸黏膜的Realtime PCR結(jié)果顯示IBD患者中Th細(xì)胞特異性轉(zhuǎn)錄因子(T-bet、GATA3、RORγ-t和Foxp3)的mRNA表達有不同程度的改變,且變化都有統(tǒng)計學(xué)意義。相關(guān)性分析顯示,外周血IL-12B與T-bet的mRNA表達呈正相關(guān)(r=0.781,P0.01);腸黏膜IL-12B與T-bet和RORγ-t的mRNA表達均呈強正相關(guān)(r=0.807,P=0.000;r=0.831,P=0.000)。血清ELISA結(jié)果顯示IL-12p40在IBD患者中的表達均高于健康對照者,且差異有統(tǒng)計學(xué)意義(P0.05)。CD患者IL-12p40表達高于UC患者(P0.05)。腸黏膜Western blot結(jié)果示IL-12p40在CD、UC患者中的表達均高于正常對照組,但只有CD患者IL-12p40蛋白的表達高于UC患者(P0.05),其受體IL-12RB1、IL-12RB2蛋白在CD、UC中表達也高于正常對照組,但差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 IL-12B mRNA和蛋白在活動期CD患者外周血及病變部位腸黏膜中表達均明顯上調(diào)。IL-12B可能通過Th1和Th17途徑在IBD患者尤其是CD患者炎癥過程中發(fā)揮重要的促炎作用。
[Abstract]:Objective to investigate the expression and significance of interleukin-12B (IL)-12B) and its receptor in peripheral blood and intestinal mucosa of patients with inflammatory bowel disease (IBD). Methods colonic mucosa specimens were collected under colonoscopy, including 60 patients with active Crohn's disease (CD), 60 patients with ulcerative colitis (UC) and 50 healthy controls. The intestinal mucosal protein levels in patients with active IBD were detected by Western blot, and those with no abnormal colonoscopy were used as controls. Plasma 2 mL, was collected from patients and healthy subjects. The total RNA, of blood cells was extracted and the mRNA expression of IL-12B and its receptor IL-12RB1 was detected by Realtime PCR method, and the expression of plasma IL-12p40 was detected by enzyme linked immunosorbent assay (ELISA). Results the results of peripheral blood Real-time PCR showed that the expression of IL-12B mRNA in patients with CD,UC was increased, and the difference was statistically significant (P0.05). The expression of IL-12RB1 mRNA was also increased in CD,UC patients, but only CD patients had statistical significance (P0.05). The results of Real-time PCR showed that the expression of IL-12B mRNA in patients with CD,UC increased significantly (P0.05), but there was no significant difference in the expression of IL-12RB1 mRNA among the three groups (P0.05). The results of Realtime PCR in peripheral blood and intestinal mucosa showed that the mRNA expression of Th cell-specific transcription factor (Th) in patients with IBD was changed to different degrees, and the changes were statistically significant (P < 0.01), but the expression of Gatt-3, ROR 緯-t and ROR 緯-t was significantly higher than that of the control group (P < 0.05). The correlation analysis showed that the expression of IL-12B in peripheral blood was positively correlated with mRNA expression of T-bet (r = 0.781, P0.01), and there was a strong positive correlation between IL-12B and mRNA expression of T-bet and ROR 緯-t in intestinal mucosa (r = 0.807, P < 0.001). (r = 0. 831, P < 0. 000). The results of serum ELISA showed that the expression of IL-12p40 in patients with IBD was significantly higher than that in healthy controls (P0.05). The expression of IL-12p40 in patients with CD was higher than that in patients with UC (P0.05). The results of Western blot showed that the expression of IL-12p40 in patients with CD,UC was higher than that in normal controls, but the expression of IL-12p40 protein in patients with CD was higher than that in patients with UC (P0.05), and its receptor IL-12RB1,IL-12RB2 protein was in CD,. The expression of UC was also higher in the control group than that in the control group, but there was no significant difference (P0.05). Conclusion the expression of IL-12B mRNA and protein is up-regulated in peripheral blood and intestinal mucosa of patients with active CD, IL-12B may play an important role in promoting inflammation in patients with IBD, especially in patients with CD through Th1 and Th17 pathway.
