MR定量分析兔肝臟脂肪含量與病理對照研究

【摘要】:目的以病理為金標(biāo)準(zhǔn),探討磁共振成像技術(shù)(正反相位DGE-MRI、質(zhì)子波譜1H-MRS)在兔非酒精性脂肪肝中定量分析的價值。 材料與方法選用40只健康雄性新西蘭兔,,隨機分為對照組(n=15)與實驗組(n=25),對照組喂食普通飼料,實驗組采用高脂飼料喂養(yǎng)建立非酒精性脂肪肝模型。喂養(yǎng)6周后,應(yīng)用DGE-MRI和1H-MRS兩種掃描方法,掃描結(jié)束后處死實驗動物,取出肝組織作病理切片,以組織病理為金標(biāo)準(zhǔn),分析肝臟脂肪含量(hepaticfat fraction,HFF)及脂水比(lipid water ratio,LWR)組間差異性,組間差異采用兩獨立樣本t檢驗;應(yīng)用ROC曲線分析HFF指數(shù)、LWR指數(shù)診斷肝細(xì)胞脂肪變(hepatic steatosis,HS)≥5%的敏感度、特異性,應(yīng)用Kappa檢驗評價DGE-MRI、1H-MRS分析肝臟脂肪變的準(zhǔn)確性。 結(jié)果1、建模成功率。普通飼料喂養(yǎng)對照組15只均無肝細(xì)胞脂肪變;高脂喂養(yǎng)引發(fā)兔非酒精性脂肪肝,實驗組25只,發(fā)生肝細(xì)胞脂肪變20只,無肝細(xì)胞脂肪變5只,建模成功率達80%(20/25)。2、普通飼料喂養(yǎng)對照組與高脂喂養(yǎng)實驗組HFF指數(shù)、LWR指數(shù)均數(shù)比較。HFF指數(shù)組間均數(shù)差異有統(tǒng)計學(xué)意義,P<0.05,對照組與實驗組均數(shù)分別為(8.710±1.886)%、(11.987±3.590)%。LWR指數(shù)組間均數(shù)差異有統(tǒng)計學(xué)意義, P<0.05,對照組與實驗組均數(shù)分別為(0.419±0.401)%、(1.684±0.995)%。3、DGE-MRI、1H-MRS診斷肝細(xì)胞脂肪變HS≥5%閾值。DGE-MRI診斷肝細(xì)胞脂肪變HS≥5%閾值9.027%,敏感度0.90,特異性0.80,AUC0.750。1H-MRS診斷肝細(xì)胞脂肪變HS≥5%閾值0.852%,敏感度1.00,特異性0.80,AUC0.860。4、DGE-MRI、1H-MRS診斷肝細(xì)胞脂肪變或脂肪肝與組織病理學(xué)診斷一致性分析。DGE-MRI Kappa值0.651,1H-MRS Kappa值0.865。兩種診斷方法與組織病理診斷均有一致性,1H-MRS與組織病理診斷一致性高于DGE-MRI。 結(jié)論雄性新西蘭兔高脂喂養(yǎng)后可成功構(gòu)建非酒精性脂肪肝模型。DGE-MRI、1H-MRS在兔非酒精性脂肪肝的定量分析中有較高價值,1H-MRS診斷效能優(yōu)于DGE-MRI。

Materials and methods Forty healthy male New Zealand rabbits were randomly divided into two groups: control group (n = 15) and experimental group (n = 25). The control group was fed with normal diet and the experimental group was fed with high fat diet to establish a non-alcoholic fatty liver model. After 6 weeks of feeding, the experimental animals were killed by DGE-MRI and 1H-MRS scanning methods. The liver tissue was taken out for pathological section. The fat content of liver was analyzed by (hepaticfat fraction, with tissue pathology as gold standard. HFF) and lipide-water ratio (lipid water ratio,LWR) were compared with each other.
