糞微生態(tài)制品移植治療小鼠實(shí)驗(yàn)性結(jié)腸炎的研究

【摘要】:目的通過對(duì)小鼠實(shí)驗(yàn)性結(jié)腸炎模型實(shí)施糞微生態(tài)制品移植(fecal microbiota transplantation, FMT)治療,探討FMT對(duì)潰瘍性結(jié)腸炎(ulcerative colitis, UC)的療效及不同供體糞微生態(tài)制品對(duì)療效有無影響。方法選用雄性BALB/c小鼠經(jīng)葡聚糖硫酸鈉(dextran sulfate sodium, DSS)誘導(dǎo)建立實(shí)驗(yàn)性結(jié)腸炎模型,將模型小鼠隨機(jī)分為對(duì)照組、自體糞微生態(tài)制品移植組、異體單一供體糞微生態(tài)制品移植組和異體混合供體糞微生態(tài)制品移植組。自體糞微生態(tài)制品移植組、異體單一供體糞微生態(tài)制品移植組和異體混合供體糞微生態(tài)制品移植組分別經(jīng)灌胃法移植自體建模前預(yù)留糞便混懸液、異體單一供體小鼠糞便混懸液和異體多只供體小鼠混合糞便混懸液,糞便混懸液均經(jīng)-80℃凍存1-2周,對(duì)照組等量生理鹽水灌胃。觀察比較小鼠一般情況、體重變化、疾病活動(dòng)指數(shù)(disease activity index, DAI)變化,取同一部位結(jié)腸觀察比較結(jié)腸病理情況,對(duì)小鼠建模前、建模后和灌胃后的糞便標(biāo)本16SrDNA的V3+V4區(qū)進(jìn)行測序分析,研究其菌群結(jié)構(gòu)變化。結(jié)果自體糞微生態(tài)制品移植組、異體單一供體糞微生態(tài)制品移植組和異體混合供體糞微生態(tài)制品移植組小鼠潰瘍性結(jié)腸炎病情均比對(duì)照組顯著好轉(zhuǎn),小鼠體重增加、DAI降低及結(jié)腸病理評(píng)分降低均與對(duì)照組有顯著差異;建模后及FMT治療后小鼠腸道菌群結(jié)構(gòu)均發(fā)生變化,自體糞微生態(tài)制品移植組經(jīng)治療后與建模前腸道菌群結(jié)構(gòu)最接近。自體糞微生態(tài)制品移植組、異體單一供體糞微生態(tài)制品移植組和異體混合供體糞微生態(tài)制品移植組小鼠體重和疾病活動(dòng)指數(shù)(DAI)無明顯差別,但異體單一供體糞微生態(tài)制品移植組和異體混合供體糞微生態(tài)制品移植組小鼠結(jié)腸病理評(píng)分高于自體糞微生態(tài)制品移植組,病理切片觀察可見異體供體糞微生態(tài)制品移植組小鼠結(jié)腸炎性細(xì)胞浸潤比自體糞微生態(tài)移植組明顯。結(jié)論FMT可通過調(diào)節(jié)腸道菌群結(jié)構(gòu)治療DSS誘導(dǎo)的小鼠實(shí)驗(yàn)性結(jié)腸炎;凍存1-2周的糞便仍然具有治療作用;FMT的治療效果與供體的腸道菌群結(jié)構(gòu)有關(guān),與患病前腸道菌群結(jié)構(gòu)越接近的供體糞便制品可能有更好的治療效果;多個(gè)異體供體的混合糞便與單一供體FMT治療效果無明顯差別。

Methods the experimental colitis model was established in male BALB/c mice induced by dextran sulfate (dextran sulfate sodium, DSS). The model mice were randomly divided into control group and autogenous faecal microecological product transplantation group. Allogeneic single donor faecal microecological product transplantation group and allogeneic mixed donor fecal microecological product transplantation group. The fecal suspension was reserved before autotransplantation of autogenous faecal microecological products, allogeneic single donor faecal microecological products transplantation group and allogeneic mixed donor faecal microecological products transplantation group, respectively.
