【摘要】：目的探討烏司他丁治療老年重癥急性胰腺炎并腹內(nèi)高壓臨床效果以及對(duì)血清學(xué)指標(biāo)影響。方法選擇80例老年重癥急性胰腺炎并腹內(nèi)高壓患者,按照數(shù)字表法隨機(jī)分為觀察組與對(duì)照組,各40例。對(duì)照組使用基礎(chǔ)治療,觀察組在對(duì)照組基礎(chǔ)上使用烏司他丁治療,對(duì)比兩組治療后有效率,治療前后腫瘤壞死因子(TNF)-α、白細(xì)胞介素(IL)-6、-10、腹內(nèi)壓(IAP)等臨床指標(biāo)。結(jié)果觀察組治療有效率為90.00%,明顯高于對(duì)照組70.00%(χ2=5.000,P0.05);兩組患者治療后TNF-α、IL-6、IL-10較治療前均有明顯改善(P0.05);觀察組治療后相比對(duì)照組改善更明顯(P0.05);兩組患者治療后IAP、免疫球蛋白(Ig)A、二胺氧化酶(DAO)相比治療前均有明顯改善(P0.05);觀察組治療后IAP、Ig A、DAO相比對(duì)照組改善更明顯(P0.05)。結(jié)論烏司他丁治療老年重癥急性胰腺炎并腹內(nèi)高壓臨床效果好,利于改善IAP以及腸黏膜功能,值得臨床推廣應(yīng)用。
[Abstract]:Objective to investigate the clinical effect of ulinastatin in the treatment of elderly severe acute pancreatitis with intraabdominal hypertension and its influence on serological indexes. Methods Eighty elderly patients with severe acute pancreatitis complicated with intraabdominal hypertension were randomly divided into observation group (n = 40) and control group (n = 40). The control group was treated with basic therapy, the observation group was treated with ulinastatin on the basis of the control group, and the effective rate was compared between the two groups. Before and after treatment, tumor necrosis factor (TNF)-偽 and interleukin (IL)-6 were used in the observation group. Intraabdominal pressure (IAP) and other clinical indicators. Results the effective rate of treatment in the observation group was 90.00g / kg, which was significantly higher than that in the control group (70.00%) (蠂 2 5.000 P 0.05), the TNF- 偽 and IL-6,IL-10 of the two groups were significantly improved after treatment (P0.05). After treatment, IAP, immunoglobulin (Ig) A, diamine oxidase (DAO) was significantly improved in the observation group than in the control group (P0.05). IAP,Ig Agnadao in the observation group was significantly improved compared with the control group (P0.05). Conclusion ulinastatin is effective in the treatment of elderly patients with severe acute pancreatitis and intraabdominal hypertension. It is beneficial to improve IAP and intestinal mucosal function and is worthy of clinical application.
