乙型肝炎病毒核心蛋白區(qū)變異與隱匿性乙型肝炎病毒感染的相關(guān)性研究

【摘要】:研究背景隱匿性乙型肝炎病毒感染(occult hepatitis B virus infection,OBI)是指患者血清HBsAg陰性,肝組織HBV DNA陽性(伴或不伴有血清HBV DNA陽性)。若患者血清中HBV DNA陽性,HBV低水平復(fù)制,HBV DNA200IU/ml。OBI不僅可以通過輸血、器官移植和透析等方式傳播HBV,而且與肝硬化和肝細(xì)胞癌的發(fā)生密切相關(guān)。乙型肝炎病毒核心蛋白具有多種功能,在pgRNA和pol多聚酶的衣殼化、核衣殼的解離和聚合、病毒顆粒的成熟中具有重要的作用。OBI的發(fā)生與HBV的基因突變密切相關(guān),現(xiàn)有的研究主要針對HBV的S區(qū)和BCP區(qū)的變異,乙型肝炎病毒核心蛋白區(qū)(C區(qū))變異在OBI的發(fā)生發(fā)展中的作用機(jī)制目前仍不清楚。研究目的探索乙型肝炎病毒核心蛋白區(qū)(C區(qū))變異在隱匿性乙型肝炎病毒感染(OBI)中的作用。研究方法本研究通過重疊延伸PCR和定點(diǎn)突變PCR技術(shù)構(gòu)建含有不同突變位的乙型肝炎病毒核心蛋白區(qū)突變型質(zhì)粒,通過細(xì)胞免疫熒光檢測肝組織HBcAg的分布情況,western blot檢測HBcAg表達(dá)水平,化學(xué)發(fā)光法檢測HBsAg和HBeAg的表達(dá)水平,免疫組織化學(xué)檢測小鼠肝組織HBcAg的表達(dá)情況,RT-PCR檢測小鼠肝組織中HBV RNA的水平。實(shí)驗(yàn)結(jié)果利用重疊PCR和定點(diǎn)突變技術(shù)成功構(gòu)建單點(diǎn)、雙聯(lián)和三聯(lián)突變的OBI毒株和野生株。HBcAg在HuH7細(xì)胞有三種不同分布類型。A類包括wt、SZB、SZA-R62W、wt-P50H、wt-L55I、wt-W62R、wt-S74G、wt-S87G、wt-L95I、wt-I97L、wt-T142M、wt-T147A、SZA-H50P/R62W、SZA-P50H/R62W/G74S、SZA-R62W/G74S、wt-P50H/L95I;B 類包括 SZA、SZA-H50P、SZA-I55L、SZA-G74S、SZA-G87S、SZA-I95L、SZA-L97I、SZA-M142T、SZA-A147T;C 類包括 wt-P50H/W62R、wt-P50H/W62R/S74G、wt-P50H/W62R/S87G、wt-P50H/W62R/T147A、wt-W62R/L95I。OBI 毒株 SZA、SZA-G87S、SZA-L97I、SZA-M142T、SZA-A147T、wt-P50H/W62R/S74G 的 HBcAg 表達(dá)水平低于野毒株,SZA-H50P、SZA-I55L、SZA-R62W、SZA-G74S、SZA-I95L 的 HBcAg 表達(dá)水平高于 SZA 的 HBcAg 表達(dá)水平。OBI毒株SZA和SZB胞內(nèi)和胞外HBeAg水平低于野毒株。OBI毒株SZA的 5 個單點(diǎn)回復(fù)突變 SZA-H50P、SZA-I55L、SZA-R62W、SZA-G74S、SZA-I95L可輕度提高胞內(nèi)和胞外HBeAg水平,SZA的4個單點(diǎn)回復(fù)突變SZA-G87S、SZA-L97I、SZA-M142T、SZA-A147T對其無明顯影響。SZA的雙聯(lián)和三聯(lián)回復(fù)突變的HBeAg表達(dá)水平要高于單點(diǎn)回復(fù)突變。野毒株的單點(diǎn)突變wt-P50H和wt-W62R 可降低 HBeAg 水平,而雙聯(lián)突變 wt-P50H/W62R、wt-W62R/L95I,三聯(lián)突變 wt-P50H/W62R/S87G、wt-P50H/W62R/T147A 的 HBeAg 表達(dá)水平低于野生型的單點(diǎn)突變。OBI毒株SZA在Balb/c小鼠肝組織不表達(dá)HBcAg,SZB在肝組織表達(dá)HBcAg。野毒株在肝組織表達(dá)HBcAg,分布類型為Ⅲ型。OBI毒株SZA的8個單點(diǎn)回復(fù)突變 SZA-H50P、SZA-I55L、SZA-G74S、SZA-G87S、SZA-L97I、SZA-M142T、SZA-A147T不表達(dá)HBcAg。單點(diǎn)回復(fù)突變SZA-R62W,雙聯(lián)回復(fù)突變 SZA-H50P/R62W、SZA-R62W/G74S,三聯(lián)回復(fù)突變 SZA-P50H/R62W/G74S可提高HBcAg的表達(dá)水平。野毒株的單點(diǎn)、雙聯(lián)、三聯(lián)突變不影響HBcAg的表達(dá)水平。Balb/c小鼠血清中,OBI毒株SZA的8個單點(diǎn)回復(fù)突變SZA-H50P、SZA-I55L、SZA-G74S、SZA-G87S、SZA-I95L、SZA-L97I、SZA-M142T、SZA-A147T的HBeAg表達(dá)水平差異不明顯,SZA的單點(diǎn)回復(fù)突變SZA-R62W能顯著提高HBeAg的表達(dá)水平。野毒株的單點(diǎn)突變wt-P50H、wt-W62R,雙聯(lián)突變 wt-P50H/W62R、wt-W62R/L95I,三聯(lián)突變 wt-P50H/W62R/S87G、wt-P50H/W62R/T147A的HBeAg表達(dá)水平低于野毒株其它位點(diǎn)的單點(diǎn)突變。第1周和第7周,SZA在小鼠肝組織始終不表達(dá)HBcAg。第1周,wt在小鼠肝臟中表達(dá)HBcAg,并且在第7周HBcAg的表達(dá)水平明顯上升。第1周,小鼠肝臟中SZA的HBV total RNA和C region RNA水平均低于野毒株。第七周,小鼠肝臟中SZA的HBV total RNA和C region RNA均高于野毒株。OBI毒株SZA的HBeAg長期處于低表達(dá)水平(10-15 S/CO)。野毒株HBeAg長期處于高表達(dá)水平(500-1500 S/CO)。OBI毒株的C區(qū)的P50H、W62R、S74G、S87G、L95I的聯(lián)合變異改變HBcAg、HBeAg、HBV RNA的表達(dá)水平,其中W62R起著最關(guān)鍵的作用。乙性肝炎病毒C區(qū)變異對OBI的形成具有重要作用。結(jié)論OBI毒株的C區(qū)的P50H、W62R、S74G、S87G、L95I的聯(lián)合變異改變HBcAg、HBeAg、HBV RNA的表達(dá)水平,其中W62R起著最關(guān)鍵的作用。乙性肝炎病毒C區(qū)變異對OBI的形成具有重要作用。

OBI is closely related to the mutation of HBV gene. Current studies mainly focus on HB. The mechanism of HBV core protein region (C) mutation in the occurrence and development of OBI is still unclear. Objective To explore the role of HBV core protein region (C) mutation in occult hepatitis B virus infection (OBI). The mutant plasmids containing different mutation sites of hepatitis B virus core protein region were constructed by mutant PCR. The distribution of HBcAg in liver tissues was detected by immunofluorescence, the expression of HBcAg was detected by Western blot, the expression of HBsAg and HBeAg was detected by chemiluminescence, and the expression of HBcAg in liver tissues of mice was detected by immunohistochemistry.
