【摘要】：目的與意義:檢測(cè)血清Ghrelin在慢性非萎縮性胃炎及慢性萎縮性胃炎中的水平,探討血清Ghrelin在不同萎縮程度及不同萎縮部位慢性萎縮性胃炎中的變化規(guī)律及意義,以期為慢性萎縮性胃炎的篩查與初步診斷提供更多的依據(jù)。研究方法:以2016年3月至2016年12月于我院消化內(nèi)鏡科門診就診,并接受電子胃鏡檢查及血清Ghrelin水平測(cè)定的慢性胃炎患者作為研究對(duì)象。根據(jù)電子胃鏡檢查及病理活檢結(jié)果,分組如下:慢性非萎縮性胃炎組、慢性萎縮性胃炎組;其中慢性萎縮性胃炎組按照萎縮程度不同可分為輕度慢性萎縮性胃炎組、中度慢性萎縮性胃炎組、重度慢性萎縮性胃炎組三組;按照萎縮部位不同可分為全胃萎縮性胃炎組、胃竇萎縮性胃炎組兩組。收集患者血清,采用酶聯(lián)免疫吸附法(Enzyme-linked immunoadsorbent assay,ELISA)檢測(cè)慢性胃炎患者的空腹血清Ghrelin水平。結(jié)果:1.慢性萎縮性胃炎組患者的血清Ghrelin水平低于慢性非萎縮性胃炎組,且慢性萎縮性胃炎各亞組的血清Ghrelin水平均較慢性非萎縮性胃炎組低,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。2.輕度慢性萎縮性胃炎組、中度慢性萎縮性胃炎組、重度慢性萎縮性胃炎組的血清Ghrelin水平不同,且血清Ghrelin水平隨著胃黏膜萎縮程度的加重而降低,各組間差異均具有統(tǒng)計(jì)學(xué)意義(P0.05);血清Ghrelin水平與胃黏膜萎縮程度有相關(guān)性,且呈負(fù)相關(guān)(r-0.593,P0.05)。3.慢性萎縮性胃炎組內(nèi),全胃萎縮性胃炎組患者的血清Ghrelin水平低于胃竇萎縮性胃炎組,且均較慢性非萎縮性胃炎組的血清Ghrelin水平低,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:血清Ghrelin水平與胃黏膜的萎縮有關(guān),且與萎縮嚴(yán)重程度呈負(fù)相關(guān),受胃黏膜萎縮部位的影響,全胃萎縮者低于胃竇萎縮者,因此血清Ghrelin可作為慢性萎縮性胃炎的篩查和初步診斷的輔助指標(biāo)之一。
[Abstract]:Objective and significance: to detect the level of serum Ghrelin in chronic non-atrophic gastritis and chronic atrophic gastritis, and to explore the changes and significance of serum Ghrelin in chronic atrophic gastritis with different atrophy degrees and different atrophy sites. In order to provide more evidence for screening and preliminary diagnosis of chronic atrophic gastritis. Methods: from March 2016 to December 2016, patients with chronic gastritis treated in our hospital from March 2016 to December 2016 were examined by electronic gastroscopy and serum Ghrelin level. According to the results of electronic gastroscopy and pathological biopsy, the groups were as follows: chronic non-atrophic gastritis group, chronic atrophic gastritis group, and chronic atrophic gastritis group can be divided into mild chronic atrophic gastritis group according to the degree of atrophy. Moderate chronic atrophic gastritis group, severe chronic atrophic gastritis group three groups, according to the atrophy position different can be divided into total gastric atrophic gastritis group, antral atrophic gastritis group two groups. The fasting serum Ghrelin levels in patients with chronic gastritis were measured by Enzyme-linked immunoadsorbent Elisa. The result is 1: 1. The serum Ghrelin level of chronic atrophic gastritis group was lower than that of chronic non-atrophic gastritis group, and the serum Ghrelin level of chronic atrophic gastritis group was lower than that of chronic non-atrophic gastritis group (P0.05). The serum Ghrelin levels were different in mild chronic atrophic gastritis, moderate chronic atrophic gastritis and severe chronic atrophic gastritis, and the serum Ghrelin levels decreased with the severity of gastric mucosal atrophy. The serum Ghrelin level was correlated with the degree of gastric mucosal atrophy, and was negatively correlated (r -0.593P 0.05). 3. In chronic atrophic gastritis group, the serum Ghrelin level of patients with total atrophic gastritis group was lower than that of antral atrophic gastritis group, and the serum Ghrelin level was lower than that of chronic non-atrophic gastritis group (P0.05). Conclusion: the level of serum Ghrelin is related to gastric mucosal atrophy and negatively correlated with the severity of atrophy. Affected by the site of gastric mucosal atrophy, the total gastric atrophy is lower than that of antral atrophy. Therefore, serum Ghrelin can be used as an auxiliary index for screening and preliminary diagnosis of chronic atrophic gastritis.
