腸結(jié)核的多層螺旋CT腸道造影表現(xiàn)

【摘要】：【目的】總結(jié)腸結(jié)核(ITB)在多層螺旋CT小腸造影(MSCTE)的表現(xiàn),探討MSCTE在ITB的診斷價(jià)值�！痉椒ā渴占�15例經(jīng)內(nèi)鏡檢查或手術(shù)后病理確診為ITB的臨床與影像學(xué)資料,所有的患者均行MSCTE檢查,回顧性分析其MSCTE表現(xiàn),包括病灶的位置、數(shù)目、形狀、邊界和鄰近組織的改變,以及腹部其它臟器、腹膜、大網(wǎng)膜和腸系膜的變化,并與內(nèi)鏡及手術(shù)結(jié)果相對(duì)照�！窘Y(jié)果】15例ITB中,13例(86%)主要累及回盲部。4例(27%)表現(xiàn)為多節(jié)段腸壁均勻增厚,9例(60%)表現(xiàn)為局部腸壁實(shí)質(zhì)性腫塊形成,1例(6%)表現(xiàn)為多發(fā)節(jié)段性腸壁增厚并腫塊形成,1例(6%)表現(xiàn)為不均勻強(qiáng)化的腫塊并腸穿孔。12例ITB出現(xiàn)淋巴結(jié)腫大,均表現(xiàn)為環(huán)形強(qiáng)化。2例(13%)ITB的腸系膜血管出現(xiàn)"梳狀征"。按其在MSCTE增強(qiáng)掃描上的強(qiáng)化模式,將該9例腫塊型小腸結(jié)核分為3種類型:a.明顯均勻強(qiáng)化型(n=5);b."靶征"征象型(n=2);c.干酪樣壞死型(n=2)�！窘Y(jié)論】ITB的影像學(xué)征象表現(xiàn)具有多樣性。MSCTE能夠清楚地顯示腸粘膜輪廓、腸壁及其與周圍組織的關(guān)系,為ITB的診斷提供豐富的信息。
[Abstract]:[objective] to summarize the manifestations of (ITB) in multislice spiral CT enterography (MSCTE) and to explore the diagnostic value of MSCTE in ITB. [methods] the clinical and imaging data of 15 cases with ITB diagnosed by endoscopy or pathology after operation were collected. All patients were examined with MSCTE. The MSCTE findings, including the location, number, shape, boundary and adjacent tissue of the lesions, and the changes of other abdominal organs, peritoneum, omentum and mesentery were analyzed retrospectively. In 13 cases (86%) of ITB, 4 cases (27%) were mainly involved in ileocecal part. 9 cases (60%) showed homogeneous thickening of multiple segments of intestine wall, and 1 case (6%) showed local solid mass of intestinal wall. In 1 case (6%) with segmental wall thickening and mass formation, 1 case (6%) showed uneven enhancement of the mass and intestinal perforation. 12 cases of ITB showed lymphadenopathy, and 1 case (6%) showed lymphadenopathy. All cases (13%) showed "comb sign" in mesenteric vessels with circular enhancement (13%). According to the enhancement pattern of MSCTE, the 9 cases of small intestinal tuberculosis with lumps were divided into three types: a. " Target sign "sign type" (n = 2). [conclusion] the imaging features of ITB are diverse. MSCTE can clearly show the outline of intestinal mucosa, the relationship between intestinal wall and surrounding tissues, and provide rich information for the diagnosis of ITB.
