【摘要】：目的分析結(jié)腸息肉大小及分布特點(diǎn),探討色素結(jié)腸鏡對(duì)結(jié)腸良性息肉的診斷及氬離子體凝固術(shù)(APC)、高頻電凝電切術(shù)、內(nèi)鏡黏膜切除術(shù)(EMR)和鈦夾術(shù)治療良性息肉的臨床療效。方法收集2015年12月-2016年6月在該院消化科住院行內(nèi)鏡及病理診斷為良性結(jié)腸息肉且行治療的175例患者資料。結(jié)果通過(guò)普通內(nèi)鏡和染色內(nèi)鏡檢查,結(jié)腸良性息肉以單發(fā)性息肉為多;結(jié)腸在多部位有多發(fā)息肉類型為多;左半結(jié)腸中的直腸和乙狀結(jié)腸息肉數(shù)較多且息肉直徑較小,右半結(jié)腸中的升結(jié)腸息肉較少且直徑較大,色素內(nèi)鏡有助于微小息肉和小息肉的診斷,染色檢查提高了結(jié)腸多發(fā)息肉的檢出率。結(jié)論色素內(nèi)鏡可以提高息肉檢出率,尤其是多發(fā)息肉檢出率,減少漏診率,有助于良性息肉的鑒別診斷;依據(jù)結(jié)腸單發(fā)和多發(fā)息肉結(jié)合息肉大小選擇APC、高頻電凝電切術(shù)或者EMR其中一種內(nèi)鏡治療或聯(lián)合2種內(nèi)鏡治療方法,療效明確、手術(shù)安全,以APC息肉切除術(shù)最常用。
[Abstract]:Objective to analyze the size and distribution of colonic polyps, and to investigate the clinical effect of colonic polyps treated by colonic colonoscopy, argon ionomer coagulation, (APC), high frequency electrocoagulation, endoscopic mucosal resection, (EMR) and titanium clip. Methods 175 cases of benign colonic polyps diagnosed by endoscopy and pathology were collected from December 2015 to June 2016. Results through common endoscopy and staining endoscopy, the benign polyps of colon were more than single polyps, the polyps of colon had more polyps in many parts, the number of polyps of rectum and sigmoid colon in left colon were more and the diameter of polyps was smaller. The polyps of ascending colon in the right hemicolon are smaller and larger in diameter. Pigment endoscopy is helpful for the diagnosis of micropolyps and small polyps. Staining can improve the detection rate of colonic polyps. Conclusion pigment endoscopy can improve the detection rate of polyps, especially the detection rate of multiple polyps, and reduce the rate of missed diagnosis, which is helpful to the differential diagnosis of benign polyps. According to the single and multiple polyps combined with the size of polyps, APC polyposis is the most commonly used method in the treatment of APC polyps.
