【摘要】：正患者男,41歲,因"上腹脹痛1天"入院。肝功能、凝血、血常規(guī)、生化、大小便常規(guī)檢查均未見異常。胃鏡提示:胃底靜脈重度曲張;食管靜脈輕度曲張。腹部CTA提示:慢性胰腺炎并周圍包裹性積液/假性囊腫形成;脾腫大�？紤]患者存在門靜脈高壓癥表現(xiàn),擬行經(jīng)頸靜脈肝內(nèi)門體分流術(shù)(TIPS)。追問病史,患者2年前有急性胰腺炎病史,曾有1次黑便,經(jīng)保守治療后好轉(zhuǎn)。近兩年來多次因腹痛住院治療�；仡櫥颊弑驹�2014
[Abstract]:The patient, 41 years old, was admitted to hospital because of "upper abdominal distension pain for 1 day". Liver function, coagulation, blood routine, biochemistry, routine examination of urine and feces were not abnormal. Gastroscopy hint: gastric fundus vein severe varices; esophageal vein mild varices. Abdominal CTA suggests chronic pancreatitis with surrounding effusion / pseudocyst formation; splenomegaly. Considering the presence of portal hypertension, intrahepatic portosystemic shunt (tips) via jugular vein was proposed. The patient had a history of acute pancreatitis 2 years ago, had a black stool once, and improved after conservative treatment.
