【摘要】：目的:探討潰瘍性結(jié)腸炎(UC)患者腸道菌群失調(diào)、血清免疫球蛋白水平及疾病活動度的相關(guān)性,確定糞便球桿比對評估UC患者菌群失調(diào)的臨床價(jià)值。方法:選取2013年1月至2015年5月北京大學(xué)第一醫(yī)院就診的401例UC患者為研究對象,根據(jù)Mayo評分分為緩解期、活動期輕、中、重度4組,回顧糞便球桿比、血清免疫球蛋白(IgG、IgM、IgA、C3、C4)檢測結(jié)果,根據(jù)糞便球桿比對菌群失調(diào)分度,分析病情活動度、腸道菌群失調(diào)、血清免疫球蛋白的相關(guān)性。結(jié)果:隨疾病活動度升高,菌群失調(diào)加重(Spearman相關(guān)系數(shù)=0.295,P0.05),血清IgM下降,C3升高,C4在輕中度活動組顯著升高。菌群失調(diào)程度與血清免疫球蛋白相關(guān),隨腸道菌群失調(diào)加重,血清IgM、IgA下降,C3上升(P0.05)。結(jié)論:腸道菌群失調(diào)與UC患者的疾病活動度相關(guān),且與機(jī)體體液免疫紊亂有關(guān),通過檢測糞便球桿比、免疫球蛋白及補(bǔ)體,對UC患者病情判斷及治療有指導(dǎo)意義。
[Abstract]:Objective: to investigate the correlation among intestinal flora disorder, serum immunoglobulin level and disease activity in patients with ulcerative colitis (UC), and to determine the clinical value of stool club ratio in evaluating dysbacteriosis in UC patients. Methods: 401 UC patients in the first Hospital of Peking University from January 2013 to May 2015 were selected as study subjects. According to Mayo score, 401 UC patients were divided into 4 groups: remission period, light, moderate and severe stage. The results of serum immunoglobulin (IgG) were analyzed according to the ratio of fecal ball rod to microflora, and the correlation of disease activity, intestinal dysbacteriosis and serum immunoglobulin was analyzed. Results: with the increase of disease activity, the dysbacteriosis increased (Spearman correlation coefficient 0.295), and serum IgM decreased and C3 increased significantly in mild to moderate activity group. The degree of dysbacteriosis was related to serum immunoglobulin. With the aggravation of intestinal dysbacteriosis, serum IgA decreased and C3 increased (P0.05). Conclusion: intestinal dysbacteriosis is related to the disease activity of UC patients and to the humoral immune disorder. The detection of stool ratio, immunoglobulin and complement may be helpful to the diagnosis and treatment of UC patients.
