【摘要】：目的：潰瘍性結(jié)腸炎（Ulcerative Colitis，UC）是一種慢性復(fù)發(fā)型疾病，近年來我國病例數(shù)量逐年上升，由于臨床及科研工作的迫切需求，潰瘍性結(jié)腸炎病例數(shù)據(jù)的整理、存儲工作就變得愈加重要。由于過去臨床醫(yī)生常用的Excel表格的局限性，臨床病例數(shù)據(jù)的存儲、整理就需要借助于更加專業(yè)化、科學(xué)化的數(shù)據(jù)庫管理系統(tǒng)（Database Management System，DBMS）的幫助。本研究的目的是如何利用國內(nèi)臨床最常用的數(shù)據(jù)庫管理系統(tǒng)——Microsoft Access設(shè)計并建立潰瘍性結(jié)腸炎Access數(shù)據(jù)庫，從而更加有效、合理地存儲、整理、分析潰瘍性結(jié)腸炎患者的臨床病例數(shù)據(jù)資料，為臨床及科研工作提供支持。 方法：1.根據(jù)潰瘍性結(jié)腸炎的臨床特點，設(shè)計數(shù)據(jù)庫字段，涵蓋了流行病學(xué)、癥狀、體征、實驗室檢查、影像學(xué)檢查、治療方案、治療效果等各個方面的數(shù)據(jù)，盡可能全面地收錄臨床病例數(shù)據(jù)。 2.根據(jù)設(shè)計好的數(shù)據(jù)庫字段，將125條字段分別建立成為11個獨立的數(shù)據(jù)庫表，根據(jù)每一個字段的臨床意義設(shè)計其數(shù)據(jù)類型，特殊類型的數(shù)據(jù)則以科學(xué)化及減輕錄入勞動強(qiáng)度為目的，通過字段屬性列表的選項來更改屬性細(xì)節(jié)。 3.根據(jù)每個獨立的表與表之間的邏輯關(guān)系，建立表之間關(guān)系，通過表間關(guān)系將獨立的數(shù)據(jù)表合理地連接成為一體。 4.在每一個數(shù)據(jù)表的基礎(chǔ)上建立窗體，制造圖形化的用戶界面，并以整潔、美觀為目的將各個窗體集合建立導(dǎo)航窗體，同時建立密碼驗證登錄窗體。 5.根據(jù)不同功能的需要，建立相對應(yīng)的宏命令。 6.數(shù)據(jù)庫框架建立完成后，利用聯(lián)眾數(shù)字化病案瀏覽器、嘉和電子病歷平臺、中聯(lián)醫(yī)院信息系統(tǒng)（旗艦版）臨床工作部件，搜索大連醫(yī)科大學(xué)附屬第一醫(yī)院2000年1月1日以來的以“潰瘍性結(jié)腸炎”、“潰瘍性結(jié)腸炎？”及“炎癥性腸病”為主要診斷的既往及住院病例，，并逐個分析，對于滿足以腹瀉或黏液膿血便為主訴入院，輔助檢查未見結(jié)直腸以外的消化道病變的病例數(shù)據(jù)錄入本數(shù)據(jù)庫中。 結(jié)果：潰瘍性結(jié)腸炎Access數(shù)據(jù)庫框架建立完成，成功生成accdb格式文件并保存，登錄驗證、窗體打開、數(shù)據(jù)錄入等各項操作均調(diào)試成功。數(shù)據(jù)庫具備數(shù)據(jù)錄入及存儲、導(dǎo)入及導(dǎo)出、查詢及報表等多項功能，同時可對數(shù)據(jù)進(jìn)行分析，并以多種圖表形式顯示分析結(jié)果。數(shù)據(jù)庫可直接應(yīng)用于臨床病例的存儲、整理工作及科學(xué)研究工作，也可作為網(wǎng)絡(luò)共享和教學(xué)工作的輔助工具。 結(jié)論：1. Microsoft Access2010數(shù)據(jù)庫管理系統(tǒng)可應(yīng)用于建立潰瘍性結(jié)腸炎臨床病例數(shù)據(jù)庫。 2.本數(shù)據(jù)庫具有數(shù)據(jù)全面、功能完善、結(jié)構(gòu)清晰、界面整潔美觀、安全性高等特點。 3.本數(shù)據(jù)庫為潰瘍性結(jié)腸炎臨床病例的收集、整理、存儲、分析等工作提供了強(qiáng)大而有力的輔助和支持。
[Abstract]:Objective: ulcerative colitis (UC) is a chronic recurrent disease. In recent years, the number of cases in China has increased year by year. Due to the urgent need of clinical and scientific research, the storage of ulcerative colitis data becomes more and more important. Due to the limitation of Excel tables commonly used by clinicians in the past, the storage of clinical case data needs the help of a more specialized and scientific database management system (DBMS). The purpose of this study is how to design and establish Access database for ulcerative colitis by using the most commonly used database management system, Microsoft Access, in order to store and organize ulcerative colitis more effectively and reasonably. To analyze the clinical data of patients with ulcerative colitis and provide support for clinical and scientific research. Method 1: 1. According to the clinical characteristics of ulcerative colitis, the database field is designed to cover the data of epidemiology, symptoms, signs, laboratory examination, imaging examination, treatment plan, therapeutic effect, etc. Collect clinical case data as fully as possible. 2. According to the designed database fields, 125 fields are established into 11 separate database tables, and their data types are designed according to the clinical significance of each field. Special types of data for the purpose of scientific and reduce the labor intensity of input, through the field property list options to change the property details. 3. According to the logical relationship between each independent table and the table, the relationship between the tables is established, and the independent data table is reasonably joined into one by the relationship between the tables. 4. 4. Create form on the basis of each data table, create graphical user interface, and set up navigation form for the purpose of neatness and beauty, and establish password verification login form. 5. According to the needs of different functions, build the corresponding macro command. 6. After the establishment of the database framework, the clinical work components of the Zhonglian Hospital Information system (flag edition) were made up of the joint digital medical record browser, the Jiahe electronic medical record platform and the Zhonglian Hospital Information system (flagship version). Search Dalian Medical University affiliated to the first hospital since January 1, 2000 with "ulcerative colitis", "ulcerative colitis?" And "inflammatory bowel disease" as the main diagnosis of the past and hospitalized cases, and analysis one by one, for the satisfaction of diarrhea or mucus blood as the main complaint of admission, auxiliary examination of the case without colorectal lesions of the digestive tract data entered into this database. Results: the Access database framework of ulcerative colitis was set up, the accdb format file was generated and saved successfully, the logon verification, the form opened and the data input were all debugged successfully. The database has many functions, such as data input and storage, import and export, query and report, etc. At the same time, the data can be analyzed, and the analysis results can be displayed in a variety of chart forms. The database can be directly applied to the storage, arrangement and scientific research of clinical cases, and it can also be used as a supplementary tool for network sharing and teaching. Conclusion 1. Microsoft Access2010 Database Management system can be used to establish a clinical case database of ulcerative colitis. 2. This database has the characteristics of comprehensive data, perfect function, clear structure, clean and beautiful interface, high security and so on. 3. This database provides powerful assistance and support for the collection, sorting, storage and analysis of clinical cases of ulcerative colitis.
