【摘要】：目的檢測(cè)急性胰腺炎(acute pancreatitis,AP)患者血漿組織因子途徑抑制劑(tissue factor pathway inhibitor,TFPI)水平,并評(píng)價(jià)其診斷重癥急性胰腺炎(severe acute pancreatitis,SAP)的臨床價(jià)值。方法取68名急性胰腺炎患者,包括SAP患者32例,輕型急性胰腺炎(mild acute pancreatitis,MAP)患者36例;另取同期門診體檢者20例為對(duì)照,均入院1 h內(nèi)抽血樣,收集臨床及實(shí)驗(yàn)室參數(shù),計(jì)算APACHEⅡ評(píng)分和Ranson評(píng)分;采用酶聯(lián)免疫吸附測(cè)定(ELISA)方法檢測(cè)血漿TFPI水平。結(jié)果 MAP組患者血漿TFPI濃度[(3026.81±465.76)pg/ml]較對(duì)照組明顯升高[(2468.73±262.39)pg/ml](P0.05),SAP患者血漿TFPI濃度[(4274.25±639.83)pg/ml]較MAP組明顯升高(P0.05)。相關(guān)分析顯示,AP患者的血漿TFPI水平與白細(xì)胞計(jì)數(shù)(WBC)、天冬氨酸轉(zhuǎn)氨酶(AST)、丙氨酸轉(zhuǎn)氨酶(ALT)、總膽紅素(TBIL)、肌酐(Cr)、凝血酶原時(shí)間(PT)、活化部分凝血活酶時(shí)間(APTT)、降鈣素原(PCT)、APACHEⅡ評(píng)分和Ranson評(píng)分呈顯著正相關(guān)(P0.05),與PLT呈顯著負(fù)相關(guān)(P0.05)。ROC曲線圖分析顯示,血漿TFPI水平診斷SAP發(fā)生的價(jià)值A(chǔ)UCSAP=0.902,95%CI=0.845~0.959(P0.05),診斷界值為血漿TFPI濃度4028.83 pg/ml時(shí),敏感度為87%,特異度為78%。結(jié)論 SAP患者的血漿TFPI水平可較正常人群和MAP患者顯著升高,并與胰腺炎危重程度顯著相關(guān),可用于診斷SAP。
