【摘要】：目的內(nèi)鏡下硬化治療是食管靜脈曲張破裂出血的重要治療方式,縱隔相關(guān)并發(fā)癥是其嚴(yán)重并發(fā)癥之一;探討縱隔相關(guān)并發(fā)癥患者的臨床特點(diǎn)。方法分析該中心2014年1月-2016年5月行食管靜脈曲張內(nèi)鏡下硬化治療患者的臨床資料,結(jié)合相關(guān)文獻(xiàn),以期了解內(nèi)鏡下硬化治療并發(fā)縱隔相關(guān)并發(fā)癥的臨床特點(diǎn)。結(jié)果共有140例食管靜脈曲張行硬化治療(男89例,女51例),均無術(shù)中明顯活動(dòng)性出血,其中發(fā)生縱隔相關(guān)并發(fā)癥2例,分別為縱隔胸膜瘺和縱隔炎并血性胸腔積液。并發(fā)癥早期發(fā)現(xiàn)后經(jīng)積極治療預(yù)后尚可。結(jié)論食管靜脈曲張破裂出血行內(nèi)鏡硬化治療療效可,存在縱隔相關(guān)并發(fā)癥可能,一般與多次操作、操作過程中注射針形成血管對穿等因素相關(guān)。
[Abstract]:Objective Endoscopic sclerotherapy is an important treatment for esophageal variceal bleeding, mediastinal complications are one of the serious complications, and the clinical features of patients with mediastinal complications are discussed. Methods the clinical data of patients with esophageal variceal endoscopic sclerosis from January 2014 to May 2016 were analyzed and the clinical features of endoscopic sclerosis with mediastinal complications were analyzed. Results there were 140 cases of esophageal varices treated with sclerotherapy (89 males and 51 females). There was no significant active bleeding during operation. Mediastinal complications occurred in 2 cases, including mediastinal pleural fistula and mediastinal inflammation with hemorrhagic pleural effusion. After early detection of complications, the prognosis was good after active treatment. Conclusion esophageal variceal bleeding can be treated by endosclerotic sclerosing. Mediastinal complications may be associated with multiple operations and angioplasty with needle injection during operation.
