【摘要】：目的探討胃鏡引導(dǎo)下聯(lián)合注射腎上腺素與噴灑超微大黃粉治療非靜脈曲張性上消化道出血(NVUGH)的臨床療效。方法選取2012年1月-2014年12月于該院接受治療的80例非靜脈曲張性上消化道出血患者作為研究對(duì)象,回顧性分析其臨床資料,按治療方案分為對(duì)照組(n=38)和觀察組(n=42)。所有患者均給予禁食、補(bǔ)充血容量和抑制胃酸等常規(guī)治療。對(duì)照組在胃鏡引導(dǎo)下給予腎上腺素注射治療,觀察組則給予胃鏡引導(dǎo)下注射腎上腺素聯(lián)合噴灑超微大黃粉治療。觀察比較兩組患者治療療效、止血時(shí)間、住院時(shí)間、再出血率及不良反應(yīng)發(fā)生情況。結(jié)果治療后,觀察組的治療總有效率為97.6%,明顯高于對(duì)照組78.9%的總有效率,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組止血時(shí)間與住院時(shí)間分別為(1.5±0.5)d與(8.2±3.2)d,均明顯短于對(duì)照組患者,差異均有統(tǒng)計(jì)學(xué)意義(均P0.05)。觀察組再出血率4.8%,明顯低于對(duì)照組再出血率23.7%(P0.05)。治療過程中,兩組患者均未發(fā)生嚴(yán)重不良反應(yīng),觀察組不良反應(yīng)發(fā)生率為23.8%,與對(duì)照組21.0%的不良反應(yīng)發(fā)生率比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論胃鏡引導(dǎo)下聯(lián)合注射腎上腺素與噴灑超微大黃粉治療非靜脈曲張性上消化道出血療效可靠,治療有效率高,止血時(shí)間與住院時(shí)間短,再出血率低,且不良反應(yīng)輕,臨床上值得進(jìn)一步研究。
[Abstract]:Objective to investigate the clinical effect of combined injection of epinephrine and ultrafine rhubarb powder under gastroscopic guidance in the treatment of non-varicose upper gastrointestinal hemorrhage (NVUGH). Methods from January 2012 to December 2014, 80 patients with non-varicose upper gastrointestinal bleeding were selected as the study objects. The clinical data were retrospectively analyzed. The patients were divided into two groups: control group (n = 38) and observation group (n = 42). All patients were given fasting, blood volume supplement and gastric acid suppression routine treatment. The control group was treated with epinephrine injection under gastroscopy, while the observation group was treated with epinephrine injection and ultrafine rhubarb powder spraying under gastroscopic guidance. The curative effect, hemostatic time, hospitalization time, rebleeding rate and adverse reaction were observed and compared between the two groups. Results after treatment, the total effective rate of the observation group was 97.6, which was significantly higher than that of the control group (78.9%). The difference was statistically significant (P0.05). The hemostasis time and hospitalization time in the observation group were (1.5 鹵0.5) d and (8.2 鹵3.2) d, respectively, which were significantly shorter than those in the control group (P0.05). The rebleeding rate in the observation group was 4.8, which was significantly lower than that in the control group (23. 7%) (P0.05). In the course of treatment, no serious adverse reactions occurred in the two groups, the incidence of adverse reactions in the observation group was 23.8. there was no significant difference between the two groups in the incidence of adverse reactions (P 0.05) compared with 21.0% in the control group (P0.05). Conclusion the combined injection of epinephrine and ultrafine rhubarb powder under gastroscopic guidance is effective and effective in the treatment of non-varicose upper gastrointestinal bleeding. The hemostasis time and hospital stay are short, the rate of rebleeding is low, and the adverse reaction is mild. It is worthy of further study clinically.
