【摘要】：目的:系統(tǒng)評(píng)價(jià)肝硬化合并門靜脈血栓形成的相關(guān)危險(xiǎn)因素,為肝硬化患者門靜脈血栓的防治提供參考依據(jù)。方法:計(jì)算機(jī)檢索PubMed、Cochrane Library、EMBASE、Science Direct、中國(guó)知網(wǎng)、維普、CBM、萬(wàn)方數(shù)據(jù)庫(kù)中與肝硬化合并門靜脈血栓形成相關(guān)的中、英文文獻(xiàn),并輔以人工檢索、文獻(xiàn)追溯等方法同時(shí)檢索,檢索時(shí)間截止到2016年8月。之后由兩位研究者獨(dú)立對(duì)所選文獻(xiàn)進(jìn)行數(shù)據(jù)提取及質(zhì)量評(píng)價(jià),并采用Stata 14.0軟件進(jìn)行數(shù)據(jù)分析。結(jié)果:所得文獻(xiàn)經(jīng)過(guò)去重、通讀全文逐步剔除后,最終納入21項(xiàng)符合標(biāo)準(zhǔn)的病例對(duì)照研究,共2854例患者。其中發(fā)生門靜脈血栓的患者871例,血栓發(fā)生率為30.6%。Meta分析結(jié)果顯示:肝硬化門靜脈血栓的形成與門靜脈流速(WMD=-1.725,95%CI:-2.064~-1.386)、門靜脈主干直徑(WMD=1.684,95%CI:1.486~1.882)、脾臟厚度(WMD=6.478,95%CI:5.580~7.376)、血小板(WMD=-8.605,95%CI:-10.797~-6.414)、血漿D-二聚體水平(WMD=2.488,95%CI:2.088~2.889)、PTHR G20210A基因(OR=4.586,95%CI:1.881~11.182)以及糖尿病(WMD=1.833,95%CI:1.291~2.603)等有關(guān);與患者的性別、年齡、肝功能、吸煙、飲酒、高血壓、凝血酶原時(shí)間、白細(xì)胞計(jì)數(shù)等無(wú)關(guān)。結(jié)論:在本Meta分析納入的參數(shù)中,脾臟厚度、門靜脈流速、門靜脈主干直徑、血小板計(jì)數(shù)、血漿D-二聚體水平、PTHR G20210A基因、糖尿病史是肝硬化合并門靜脈血栓形成的主要因素,而患者基本狀況、肝功、凝血酶原時(shí)間等指標(biāo)與門靜脈血栓形成無(wú)直接相關(guān)性。
[Abstract]:Objective: to evaluate the risk factors of portal vein thrombosis in cirrhotic patients and to provide reference for prevention and treatment of portal vein thrombosis in cirrhotic patients. Methods: the Chinese and English literatures related to portal vein thrombosis associated with cirrhosis and portal vein thrombosis were searched by computer in PubMedus Cochrane Library EMBASE Science DirectNet, CBM, Wanfang database, and were retrieved simultaneously by manual search and literature tracing. The search time is up to August 2016. After that, two researchers carried out the data extraction and quality evaluation independently, and analyzed the data by Stata 14.0 software.
