【摘要】：目的:檢測分析急性胰腺炎(AP)患者外周血中性粒細胞明膠酶相關(guān)脂質(zhì)運載蛋白(NGAL)的動態(tài)變化及其臨床意義。方法:選取收治的117例AP患者,依據(jù)病情分為急性輕癥胰腺炎(輕癥組)60例,急性重癥胰腺炎(重癥組)57例;同時選取體檢中心收集的健康自愿者60例作為健康組,分別檢測三組研究對象的NGAL等指標(biāo)并進行統(tǒng)計分析。結(jié)果:在入院后第1、3、7天重癥組患者的外周血抵抗素、hs-CRP、NGAL、血淀粉酶、PCT測定值均顯著的高于輕癥組和健康組(P0.05);輕癥組患者入院后第1、3天的外周血抵抗素、hs-CRP、NGAL、血淀粉酶、PCT測定值顯著的高于健康組(P0.05);輕癥組患者入院后第7天的血淀粉酶、PCT測定值顯著的高于健康組(P0.05)。重癥組患者的APACHE II評分在入院后第1、3、7天均顯著的高于輕癥組患者(P0.05)。重癥組患者的Balthazar CT評分在入院后第1、7天均顯著的高于輕癥組患者(P0.05)。AP患者外周血NGAL水平與其外周血抵抗素、hs-CRP、PCT、PACHE II評分、Balthazar CT評分均呈正相關(guān)(P0.05)。結(jié)論:NGAL與AP患者病情嚴重程度具有顯著關(guān)系,可作為臨床治療重要的參考指標(biāo)。
[Abstract]:Objective: to detect and analyze the dynamic changes and clinical significance of neutrophil gelatinase related lipid carrier protein (NGAL) in peripheral blood of patients with acute pancreatitis (AP) and its clinical significance. Methods: 117 cases of AP patients were selected and divided into 60 cases of acute mild pancreatitis (light disease group) and 57 cases of severe acute pancreatitis (severe group). 60 healthy volunteers were used as the health group to test the NGAL and other indexes of three groups of subjects. Results: the peripheral blood resistin, hs-CRP, NGAL, blood amylase and PCT were significantly higher than those of the mild group and the healthy group (P0.05) at the first 1,3,7 day after admission, and the patients in the light group were given 1,3 days after admission. The values of peripheral blood resistin, hs-CRP, NGAL, blood amylase and PCT were significantly higher than that in the healthy group (P0.05). The serum amylase in the light syndrome group was significantly higher than that in the healthy group (P0.05) at seventh days after admission. The APACHE II score in the severe group was significantly higher than that in the light group (P0.05) at the 1,3,7 day after admission. The Azar CT score was significantly higher than that of the patients in the light disease group (P0.05) on day 1,7. The level of peripheral blood NGAL in the peripheral blood of the patients with.AP was positively correlated with the peripheral blood resistin, hs-CRP, PCT, PACHE II score and Balthazar CT score.
