【摘要】：目的:探討應(yīng)用實(shí)時(shí)剪切波彈性成像(real-time shear wave elastography,SWE)在慢性乙型肝炎肝纖維化程度評(píng)價(jià)中的應(yīng)用價(jià)值。方法:將SWE應(yīng)用于60例健康對(duì)照者、68例慢性乙型肝炎患者行肝臟彈性模量檢測(cè),并對(duì)68例慢性乙肝患者進(jìn)行肝穿刺活組織檢查,以病理檢查結(jié)果的纖維化分期與肝臟的SWE測(cè)值進(jìn)行比較。結(jié)果:健康對(duì)照組與肝纖維化組的彈性模量值比較差異有統(tǒng)計(jì)學(xué)意義,慢性乙型肝炎肝纖維化分期S1期、S2期、S3期、S4期的彈性模量值分別為(7.171±1.819)k Pa(n=14)、(10.300±3.121)k Pa(n=18)、(15.540±4.341)k Pa(n=20)、(23.919±5.698)k Pa(n=16)。各病理分期間彈性模量差異具有統(tǒng)計(jì)學(xué)意義(F=50.807,P0.001)。肝彈性模量與肝纖維化病理分期具有相關(guān)性(r=0.783,P0.001)。以肝彈性模量判斷肝纖維化分期繪制受試者操作特性曲線,肝硬化分期≥S2、≥S3和S4的受試者工作特征曲線下面積分別為0.940、0.947和0.949。結(jié)論:SWE技術(shù)對(duì)慢性乙型肝炎肝纖維化程度有一定的診斷效能,具有潛在的臨床實(shí)用價(jià)值。
[Abstract]:Objective: to evaluate the value of real-time shear wave elastography in evaluating the degree of liver fibrosis in chronic hepatitis B patients. Methods: SWE was applied to 68 patients with chronic hepatitis B and 68 patients with chronic hepatitis B. Liver biopsy was performed in 68 patients with chronic hepatitis B. The fibrosis stage of pathological examination was compared with the SWE value of liver. Results: there was significant difference in the elastic modulus between the healthy control group and the hepatic fibrosis group. The elastic modulus of stage S4 in stage S3 was (7.171 鹵1.819) KPA (n = 14), (鹵3.121) KPA (n = 18), (15.540 鹵4.341) KPA (n 20), (23.919 鹵5.698) KPA (n ~ (18), (15.540 鹵4.341) KPA (n ~ (20), (23.919 鹵5.698) KPA (n ~ (18), (鹵4.341) KPA (n ~ (20), (23.919 鹵5.698) KPA respectively. There was significant difference in elastic modulus among pathological stages (FV 50.807 P 0.001). There was a correlation between the elastic modulus of liver and the pathological stage of hepatic fibrosis (r = 0.783, P 0.001).
