【摘要】：目的:觀察慢性丙型肝炎（CHC）患者血清中的白介素-18（IL-18）、白介素-4（IL-4）的水平及其與丙型肝炎病毒載量（HCV RNA）、丙氨酸氨基轉(zhuǎn)移酶（ALT）的關(guān)系，探討IL-18、IL-4在CHC患者HCV病毒復(fù)制及致病機(jī)制中可能的作用。 方法:收集2012年12月至2013年4月在廣西醫(yī)科大學(xué)第一附屬醫(yī)院就診的CHC患者59例，健康對照組20例，所有CHC患者均符合2004年中華醫(yī)學(xué)會(huì)肝病學(xué)分會(huì)聯(lián)合傳染病與寄生蟲病學(xué)分會(huì)發(fā)布的《丙型肝炎防治指南》中CHC診斷標(biāo)準(zhǔn)。采用ELISA法、PCR-熒光探針法、全自動(dòng)生化分析儀分別檢測研究對象外周血清IL-18、IL-4、HCV RNA、ALT等指標(biāo)。 結(jié)果: （1）CHC患者組與健康對照組相比較，IL-18表達(dá)水平顯著升高，，兩組中位數(shù)分別為118.52pg/mL、11.09pg/mL，差異具有統(tǒng)計(jì)學(xué)意義（P=0.000）；IL-4表達(dá)水平在病例組與健康組間差異無統(tǒng)計(jì)學(xué)意義（P＞0.05）；IL-18/IL-4在CHC患者組中（M=28.27pg/mL）高于健康對照組（M=2.02pg/mL），差異具有統(tǒng)計(jì)學(xué)意義（P＜0.05）。 （2）CHC患者組IL-18與IL-4兩變量進(jìn)行相關(guān)性分析，結(jié)果示兩變量呈正相關(guān)（r=0.615）。 （3）IL-18、IL-4表達(dá)在HCV RNA陽性組與陰性組間差異無統(tǒng)計(jì)學(xué)意義（P＞0.05）。 （4）HCV RNA陽性患者病毒載量與IL-18、IL-4濃度分別進(jìn)行相關(guān)性分析，結(jié)果均無顯著相關(guān)性，r值分別為0.015、0.175。 （5）病例組血清ALT水平與IL-18、IL-4水平分別進(jìn)行相關(guān)性分析，結(jié)果無明顯相關(guān)性，r值分別為0.191、0.235。 結(jié)論: （1）慢性丙型肝炎疾病過程中IL-18明顯升高，IL-18與疾病的發(fā)展存在一定的關(guān)系； （2）慢性丙型肝炎患者IL-18/IL-4失衡，Th1細(xì)胞優(yōu)勢可能在其發(fā)病中發(fā)揮一定的作用。
[Abstract]:Objective: to observe the serum levels of interleukin-18 (IL-18) and interleukin-4 (IL-4) in patients with chronic hepatitis C (CHCC) and their relationship with HCV RNA and alanine aminotransferase (alt). To explore the possible role of IL-18 IL-4 in HCV virus replication and pathogenesis in CHC patients. Methods: from December 2012 to April 2013, 59 CHC patients in the first affiliated Hospital of Guangxi Medical University and 20 healthy controls were collected. All patients with CHC were in accordance with the diagnostic criteria of CHC published in the guidelines for the Prevention and treatment of Hepatitis C, published by the Association of Hepatology and Hepatology of the Chinese Medical Association in 2004. Elisa and PCR- fluorescence probe were used to detect IL-18, IL-4, IL-4, HCV RNA, alt and so on. Results: the expression of IL-18 in CHC group was significantly higher than that in healthy control group. The median of the two groups was 118.52pg / mL 11.09pg / mL, the difference was statistically significant (P < 0.01). There was no significant difference in the expression of IL-4 between the two groups (P > 0.05). IL-18 / IL-4 in CHC group was 28.27pg / mL) higher than that in healthy control group (2.02 PG / mLL), the difference was significant (P < 0.05). The correlation between IL-18 and IL-4 in CHC group was analyzed. The results showed that there was no significant difference in the expression of IL-18 IL-4 between HCV RNA positive group and negative group (P > 0.05). Results there was no significant correlation between serum alt level and IL-18 IL-4 level. The results showed that there was no significant correlation between serum alt level and IL-18 IL-4 level. Conclusion: (1) the increase of IL-18 in patients with chronic hepatitis C has a certain relationship with the development of the disease, and the imbalance of IL-18 / IL-4 in patients with chronic hepatitis C may play a role in the pathogenesis of chronic hepatitis C.
