【摘要】：目的比較聯(lián)動(dòng)成像內(nèi)鏡(LCI)與傳統(tǒng)白光內(nèi)鏡對(duì)胃黏膜幽門螺桿菌(Hp)感染的診斷價(jià)值。方法用白光和LCI模式分別觀察胃黏膜,用取色軟件測(cè)量?jī)煞N模式下可疑Hp感染區(qū)域的紅色(R)、綠色(G)、藍(lán)色(B)的組成值,并對(duì)可疑部位取活檢行病理檢測(cè),R/(G+B)比值用于建立預(yù)測(cè)Hp感染的受試者工作特征曲線(ROC),并計(jì)算曲線下面積(AUC)、cut-off值及靈敏度和特異度。結(jié)果共納入47例研究對(duì)象,其中維吾爾族23例,平均49歲。共獲得91例病理組織,確診Hp感染44例。LCI模式建立的曲線的AUC為0.616,cut-off值為0.967,靈敏度為0.955,特異度為0.298;白光模式建立的曲線的AUC為0.529,cut-off值為2.638,靈敏度為0.455,特異度為0.766。LCI診斷漢族和維吾爾族Hp感染的AUC分別為0.65、0.549。LCI診斷萎縮性胃炎和非萎縮性胃炎患者Hp感染的AUC分別為0.628、0.603。結(jié)論 LCI模式較白光模式診斷胃黏膜Hp感染具有更高的診斷效能,可作為一種客觀的內(nèi)鏡下輔助判斷胃黏膜Hp感染的定量方法。
[Abstract]:Objective to compare the diagnostic value of combined imaging endoscopy (LCI) and traditional white light endoscopy in the diagnosis of Helicobacter pylori (HP) infection in gastric mucosa. Methods the gastric mucosa was observed by white light and LCI mode, and the composition values of the suspected HP infection areas were measured by color extraction software. The ratio of R- / G to G) was used to establish the operating characteristic curve for predicting HP infection, and the area under the curve was calculated as a cut-off value, sensitivity and specificity. Results A total of 47 subjects were included, 23 of them were Uygur, with an average age of 49 years. 91 cases of pathological tissues were obtained. The AUC value, sensitivity and specificity of the curve established by the LCI model in 44 cases with confirmed HP infection were 0.616 cut-off value 0.967, the sensitivity was 0.955, the specificity was 0.298, the AUC value of the curve established by the white-light model was 0.529 cut-off value was 2.638, the sensitivity was 0.455.The specificity of the curve was 0.766.LCI for the diagnosis of HP infection in Han and Uygur nationalities. The AUC of HP infection in patients with atrophic gastritis and non-atrophic gastritis was 0.628 and 0.603, respectively. Conclusion LCI model is more effective than white light mode in the diagnosis of HP infection in gastric mucosa. It can be used as an objective quantitative method for the diagnosis of HP infection in gastric mucosa under endoscope.
