【摘要】：目的系統(tǒng)評(píng)價(jià)大黃與奧曲肽聯(lián)用治療急性胰腺炎的有效性和安全性。方法計(jì)算機(jī)檢索中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)(CBM)、維普(VIP)、中國(guó)知網(wǎng)(CNKI)、PubMed和萬方數(shù)據(jù)庫(kù),查找有關(guān)應(yīng)用奧曲肽與大黃聯(lián)用治療急性胰腺炎的隨機(jī)對(duì)照試驗(yàn)(RCT),并追索納入文獻(xiàn)的參考文獻(xiàn),檢索時(shí)限:2008年1月至2014年12月。英文檢索詞為"Octreotide","rhubarb","ASP",中文檢索詞為"大黃"、"奧曲肽"、"急性胰腺炎"。2位評(píng)價(jià)員按照Corchrane系統(tǒng)評(píng)價(jià)方法,預(yù)先制定納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn),對(duì)檢索的文獻(xiàn)進(jìn)行獨(dú)立篩選、數(shù)據(jù)提取和文獻(xiàn)評(píng)估,交叉核對(duì)相關(guān)數(shù)據(jù),使用RevMan 5.2軟件分析。結(jié)果納入10篇RCT,共655例受試者(試驗(yàn)組336例,對(duì)照組319例)。Meta分析結(jié)果顯示:與單用奧曲肽相比,大黃聯(lián)合奧曲肽可以提高顯效率[RR=1.47,95%CI(1.21,1.78),P0.01]和總有效率[RR=1.19,95%CI(1.09,1.29),P0.01];試驗(yàn)組與對(duì)照組在縮短腹痛時(shí)間、血淀粉酶恢復(fù)時(shí)間、排便恢復(fù)時(shí)間、患者住院時(shí)間、腹脹消失時(shí)間、嘔吐消失時(shí)間、尿淀粉酶恢復(fù)時(shí)間、腹壓痛消失時(shí)間方面,均差異有統(tǒng)計(jì)學(xué)意義(P0.01);可降低轉(zhuǎn)手術(shù)率,但差異無統(tǒng)計(jì)學(xué)意義(P0.05);兩組安全性方面未見嚴(yán)重不良反應(yīng)報(bào)道。結(jié)論與單用奧曲肽相比,奧曲肽聯(lián)合大黃治療急性重癥胰腺炎能提高奧曲肽的臨床療效,安全性較好。
[Abstract]:Objective to evaluate the efficacy and safety of combined use of rhubarb and octreotide in the treatment of acute pancreatitis. Methods computer retrieval of Chinese biomedical literature database (CBM), VIP, CNKI, PubMed and Wanfang database were used to find a randomized controlled trial (RCT) for the treatment of acute pancreatitis with octreotide and rhubarb (RCT). Recourse to the reference literature, retrieval time limit: January 2008 to December 2014. English retrieval words "Octreotide", "rhubarb", "ASP", Chinese retrieval words "rhubarb", "octreotide", "acute pancreatitis".2 evaluators in accordance with the Corchrane system evaluation method, pre formulation of the inclusion criteria and exclusion criteria, independent screening of the literature for screening. Selection, data extraction and literature evaluation, cross checking related data, using RevMan 5.2 software analysis. The results were included in 10 RCT. A total of 655 subjects (336 cases in test group and 319 control group) showed.Meta analysis. Compared with octreotide, rhubarb combined octreotide could improve the efficiency of [RR=1.47,95%CI (1.21,1.78), P0.01] and total effective [RR =1.19,95%CI (1.09,1.29), P0.01], the test group and the control group had a significant difference in shortening the time of abdominal pain, the time of blood amylase recovery, the recovery time of defecation, the time of hospitalization, the time of disappearance of abdominal distention, the time of disappearance of vomit, the time of the recovery of urine amylase, and the disappearance time of abdominal pressure pain (P0.01), which could reduce the rate of operation, but the difference was no unification. No serious adverse reactions were reported in the two groups. Conclusion compared with octreotide alone, octreotide combined with rhubarb can improve the clinical efficacy of octreotide in the treatment of acute severe pancreatitis, and the safety is better.
