【摘要】：目的探討重癥胰腺炎(SAP)患者連續(xù)腎臟替代療法(CRRT)中心靜脈導(dǎo)管封管的合理肝素濃度。方法 60例SAP采用中心靜脈置管行CRRT治療的患者隨機(jī)分為3組(n=20)。A組:低濃度肝素鹽水(625 U/m L),B組:中濃度肝素鹽水(1 250 U/m L),C組:高濃度肝素鹽水(2 500 U/m L)。監(jiān)測(cè)血管通路通暢、出血情況,測(cè)定封管前、封管后2、4 h凝血指標(biāo)。結(jié)果各組患者導(dǎo)管堵塞和出血發(fā)生情況比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。3組患者間封管前、封管后2、4 h凝血功能指標(biāo)[凝血酶原時(shí)間(PT)、活化部分凝血酶時(shí)間(APTT)、凝血酶時(shí)間(TT)]比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。3組患者封管后2、4 h凝血功能指標(biāo)與封管前比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論低濃度肝素封管液可以用于大多數(shù)SAP患者CRRT中心靜脈導(dǎo)管的封管,有高凝傾向的患者可能更適合采用高濃度肝素液。
[Abstract]:Objective to investigate the reasonable concentration of heparin in patients with severe pancreatitis (SAP) treated by continuous renal replacement therapy (CRRT). Methods Sixty patients with SAP treated with central venous catheterization CRRT were randomly divided into 3 groups: group A: low concentration heparin saline (625 U / m L) group: middle concentration heparin saline solution (1 250 U / m L); group C: high concentration heparin salt water (n = 2 500 U / m L). The blood coagulation indexes were measured before and 2 hours after closure. Results there was no significant difference in the incidence of catheterization and bleeding in all groups. There was no significant difference between the two groups. There was no significant difference in coagulation function between P0.05.3 group and P0.053.There was no significant difference in coagulation function between the two groups. Conclusion low concentration heparin can be used in the sealing of CRRT central venous catheter in most SAP patients, and the patients with high coagulation tendency may be more suitable for high concentration heparin solution.
