【摘要】：目的評估非壺腹部十二指腸病變內鏡黏膜切除術(EMR)后鈦夾閉合創(chuàng)面的臨床價值。方法回顧性分析54例十二指腸黏膜病變EMR術后患者,根據(jù)是否行鈦夾閉合創(chuàng)面分為鈦夾組(n=25)和對照組(n=29),比較兩組患者的EMR術后遲發(fā)性出血率及術后4和6周的潰瘍分期及瘢痕愈合率。結果鈦夾組患者無術后遲發(fā)性出血發(fā)生,對照組有6例,鈦夾組術后遲發(fā)性出血率顯著低于對照組(P0.05)。鈦夾組術后4和6周的瘢痕期(S1)期潰瘍瘢痕愈合率分別為88.0%和100.0%,分別顯著高于同期對照組的潰瘍瘢痕愈合率55.2%和79.3%(P0.05)。對兩組的潰瘍分期比較,鈦夾組患者術后4和6周的整體潰瘍愈合質量均顯著高于對照組患者(P0.05)。結論金屬鈦夾閉合十二指腸黏膜病變EMR術后創(chuàng)面可顯著降低術后遲發(fā)性出血的風險,并可提高醫(yī)源性潰瘍的愈合速度和愈合質量。
[Abstract]:Objective to evaluate the clinical value of titanium clip wound closure after endoscopic mucosal resection (EMR) for non-ampullary duodenal lesions. Methods 54 cases of duodenal mucosal lesions after EMR were retrospectively analyzed. According to whether the wound was closed with titanium clip or not, the patients were divided into two groups: the titanium clip group (n = 25) and the control group (n = 29). The rate of delayed bleeding after EMR, the stage of ulcer 4 and 6 weeks after operation and the rate of scar healing were compared between the two groups. Results there were no delayed bleeding in the titanium clip group and 6 cases in the control group. The rate of delayed hemorrhage in the titanium clip group was significantly lower than that in the control group (P 0.05). The rate of scar healing in the titanium clip group at 4 and 6 weeks after operation was 88.0% and 100.0% respectively, which was significantly higher than that in the control group (55.2% and 79.3% respectively). The overall ulcer healing quality in the titanium clip group was significantly higher than that in the control group at 4 and 6 weeks after operation. Conclusion closure of duodenal mucosal lesions with titanium clip can significantly reduce the risk of delayed bleeding after EMR and improve the healing speed and healing quality of iatrogenic ulcer.
