【摘要】：目的：探討慢性乙型肝炎(Chronic hepatitis B, C H B)患者不同肝臟超聲評(píng)分組之間肝大小指數(shù)、肝門靜脈內(nèi)徑及流速、肝功能指標(biāo)的差異,并分析超聲評(píng)分、肝大小指數(shù)、門靜脈內(nèi)徑及流速與肝功能指標(biāo)的相關(guān)性。 方法：對(duì)67例慢性乙型肝炎患者(男性患者43例,女性患者24例,年齡22-60歲,平均年齡48.3士9.6歲)的肝包膜、邊緣角、實(shí)質(zhì)、靜脈壁及膽囊壁回聲進(jìn)行超聲半定量評(píng)分,并測(cè)量肝大小指數(shù)、門靜脈主干內(nèi)徑、門靜脈主干血流速度。分析肝臟不同超聲評(píng)分組之間的肝功能各項(xiàng)指標(biāo)、肝臟大小指數(shù)、門靜脈內(nèi)徑及血流速度的差異；研究超聲評(píng)分、肝臟大小指數(shù)、門靜脈內(nèi)徑及血流與肝功能指標(biāo)之間的相關(guān)性。 結(jié)果：①不同超聲評(píng)分組的肝大小指數(shù)、門靜脈內(nèi)徑及流速、肝功能指標(biāo)比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。②血清總蛋白(Total Protein.TP)、清蛋白(Albumin.A l b)、前清蛋白(Prealbumin、P A)、膽堿酯酶(Cholinesterase.C H E)與超聲評(píng)分、門靜脈內(nèi)徑負(fù)相關(guān)；與肝大小指數(shù)、門靜脈流速正相關(guān)(|r|均0.4,P0.05)。③血清堿性磷酸酶(Alkaline phosphatase.A l p)與超聲評(píng)分、門靜脈內(nèi)徑正相關(guān)；與肝大小指數(shù)、門靜脈流速負(fù)相關(guān)(|r|均0.4,PO.05)。④超聲評(píng)分與TP、Alb、P A、C H E、A l p的相關(guān)系數(shù)分別為r=-0.634、r=-0.761、r=-0.877、r=-0.763、r=0.722,相關(guān)系數(shù)較其他超聲指標(biāo)高。 結(jié)論：肝臟超聲評(píng)分、肝臟大小指數(shù)、肝門靜脈內(nèi)徑及血流速度與肝功能指標(biāo)變化有一定相關(guān)性,其中肝臟超聲評(píng)分高低與肝功能關(guān)系更為密切。
[Abstract]:Objective: to study the difference of liver size index, portal vein diameter and velocity, liver function index between different liver ultrasound scoring groups in chronic hepatitis B patients with chronic hepatitis B hepatitis B (C-H B), and to analyze the ultrasonic score and liver size index. Correlation between portal vein diameter and flow velocity and liver function. Methods: the echo of liver capsule, marginal angle, parenchyma, venous wall and gallbladder wall in 67 patients with chronic hepatitis B (43 male and 24 female, aged 22-60 years, mean age 48.3 鹵9.6 years) were assessed by ultrasonic semi-quantitative method.
