【摘要】：目的:探討CT小腸造影(CTE)在評(píng)估克羅恩病活動(dòng)度方面的診斷價(jià)值。方法:收集2012年3月—2016年8月本院消化內(nèi)科住院患者,經(jīng)CTE、內(nèi)鏡或病理證實(shí)為克羅恩病(CD)的患者31例,根據(jù)Harvey-Bradshaw指數(shù)(HBI)將患者分為活動(dòng)期組(17例)和緩解期組(14例)。比較兩組在CTE表現(xiàn)和C反應(yīng)蛋白(CRP)指標(biāo)間的差異。結(jié)果:活動(dòng)期組較緩解期組有更大的小腸管壁厚度[(9.28±2.82)mm vs.(6.76±2.33)mm,P=0.012]、管壁強(qiáng)化程度[(111.47±9.8)HU vs.(91.50±9.55)HU,P=0.000]、CRP值[(48.56±31.03)mg/L vs.(20.24±19.29)mg/L,P=0.006]�；顒�(dòng)期組梳征(14例)多于緩解期組(3例),差異具有統(tǒng)計(jì)學(xué)意義(P=0.001)�；顒�(dòng)期組淋巴結(jié)增大、瘺管、蜂窩織炎及腸管狹窄的發(fā)生率(76.5%、23.5%、35.3%、100%)高于緩解期組(50.0%、7.1%、14.3%、85.7%),差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論:克羅恩病患者CTE上管壁的增厚程度、強(qiáng)化程度、強(qiáng)化方式、梳征對(duì)活動(dòng)度的評(píng)估有重要指導(dǎo)作用。
[Abstract]:Objective: to evaluate the diagnostic value of CT enterography (CTE) in evaluating the activity of Crohn's disease. Methods: from March 2012 to August 2016, 31 inpatients with Crohn's disease confirmed by CTE, endoscopy or pathology were divided into active group (n = 17) and remission group (n = 14) according to Harvey-Bradshaw index. To compare the difference of CTE and C-reactive protein (CRP) between the two groups. Results: the thickness of small intestine wall in active group was greater than that in remission group [9.28 鹵2.33 2.82)mm vs.(6.76 鹵2.33 mm P0. 012], and the degree of wall enhancement [111.47 鹵9.55 9.8)HU vs.(91.50 鹵9.55 2.82)mm vs.(6.76 鹵9.55] CRP value [48.56 鹵19.29 mg / L P 0.006]. There were 14 cases of combing sign in active stage group than 3 cases in remission stage group, the difference was statistically significant. The incidences of lymph node enlargement, fistula, cellulitis and intestinal stenosis in active group were 76.53.35 and 100) higher than that in remission group 50.07.1and 85.75.70.There was no significant difference. Conclusion: the thickening degree, strengthening degree, strengthening mode and combing sign of upper wall of CTE in patients with Crohn's disease play an important role in the evaluation of mobility.
