2011—2015年細(xì)菌性腹膜炎病原菌分布及耐藥性

【摘要】:目的了解細(xì)菌性腹膜炎病原菌分布及耐藥狀況,為臨床抗感染治療提供實驗室依據(jù)。方法收集徐州醫(yī)科大學(xué)附屬醫(yī)院2011—2015年細(xì)菌性腹膜炎患者腹腔積液培養(yǎng)分離的病原菌,對其進(jìn)行細(xì)菌鑒定和藥敏試驗,分析病原菌分布和耐藥情況。結(jié)果共收集菌株491株,其中革蘭陰性桿菌291株(59.26%),革蘭陽性球菌196株(39.92%),真菌4株(0.82%),排名居前5位的病原菌分別為大腸埃希菌(30.14%)、凝固酶陰性葡萄球菌(12.22%)、金黃色葡萄球菌(10.39%)、肺炎克雷伯菌(8.55%)和屎腸球菌(6.52%)。大腸埃希菌、肺炎克雷伯菌、鮑曼不動桿菌和銅綠假單胞菌對亞胺培南的耐藥率分別是4.90%、31.04%、77.28%和26.27%。耐甲氧西林金黃色葡萄球菌(MRSA)檢出率為56.02%,耐甲氧西林凝固酶陰性葡萄球菌(MRNCS)檢出率為70.02%。結(jié)論引起細(xì)菌性腹膜炎的病原菌以革蘭陰性桿菌為主,大腸埃希菌占首位;病原菌耐藥情況嚴(yán)重,應(yīng)該加強(qiáng)抗菌藥物的規(guī)范使用,減少耐藥菌株的產(chǎn)生。
[Abstract]:Objective to investigate the distribution and drug resistance of pathogenic bacteria in bacterial peritonitis and to provide laboratory evidence for clinical anti-infection therapy. Methods the pathogens isolated from peritoneal effusion of patients with bacterial peritonitis from 2011 to 2015 in affiliated Hospital of Xuzhou Medical University were collected. Bacterial identification and drug sensitivity test were performed to analyze the distribution and drug resistance of the pathogens. Results A total of 491 strains were collected. Among them, 291 strains of Gram-negative bacilli, 196 strains of Gram-positive coccus, 4 strains of fungi, respectively, are Escherichia coli 30.14, coagulase negative Staphylococcus 12.2222, Staphylococcus aureus 10.39, Klebsiella pneumoniae 8.5555 and Enterococcus faecium 6.52B. among them, 291 strains of Gram-negative bacilli, 196 strains of Gram-positive coccus, 4 strains of fungi, and 4 strains of fungi, respectively, are Escherichia coli 30.14, coagulase negative staphylococcus 12.22, Staphylococcus aureus 10.39, Klebsiella pneumoniae 8.5555. The resistance rates of Escherichia coli, Klebsiella pneumoniae, Acinetobacter baumannii and Pseudomonas aeruginosa to imipenem were 4.90,31.04% and 26.27%, respectively. The positive rates of MRSAs and MRNCSs of methicillin-resistant staphylococcus aureus and methicillin-resistant coagulase-negative staphylococci were 56.02 and 70.02 respectively. Conclusion Gram-negative bacilli are the main pathogenic bacteria causing bacterial peritonitis, Escherichia coli is the first, and the drug resistance of pathogenic bacteria is serious, we should strengthen the standard use of antimicrobial agents and reduce the production of drug-resistant strains.
