【摘要】：正患者男,53歲。因"惡心、嘔吐伴停止排氣、排便1天"入院�；颊�1天前無(wú)明顯誘因出現(xiàn)惡心、嘔吐,嘔吐物為胃內(nèi)容物,非噴射狀,后肛門停止排氣、排便,患者腹脹感逐漸加重,偶有陣發(fā)性疼痛,無(wú)畏寒、發(fā)熱。既往史:5年前因"壺腹部癌"行"根治性胰十二指腸切除術(shù)"。入院體檢:腹膨隆,上腹部深壓痛,腸鳴音弱,無(wú)氣過(guò)水聲,無(wú)胃腸型,右側(cè)腹直肌切口可見長(zhǎng)約25 cm手術(shù)切口瘢痕;實(shí)驗(yàn)室檢查:白細(xì)胞計(jì)數(shù)15.99×10~9/L。CT檢查:下腹部小腸腸管內(nèi)見一含氣泡的花斑樣食物團(tuán),大小約為6.2 cm×3.4 cm,相鄰腸管壁略顯增厚,周邊脂肪間隙稍模糊(圖1),食物團(tuán)完全阻塞腸管,胃及近段小腸
[Abstract]:The patient is 53 years old. Admitted to hospital for "nausea, vomiting with cessation of exhaust, defecation for 1 day". The patient had no obvious inducement of nausea, vomiting, vomit, stomach content, non-jetting, anus stop exhaust, defecation, abdominal distention gradually aggravated, occasional paroxysmal pain, no chills, fever. History: radical pancreaticoduodenectomy was performed 5 years ago because of ampullary carcinoma. Physical examination: abdominal distention, deep tenderness of upper abdomen, weak voice of bowel sound, no air overwater sound, no gastrointestinal type, scar of right rectus abdominis incision can be seen about 25 cm long; Laboratory examination: White blood cell count 15.99 脳 10~9/L.CT: a blotchy food mass containing bubbles was found in the lower abdominal small intestine, about 6.2 cm 脳 3.4 cm in size, the adjacent intestinal wall was slightly thickened, and the peripheral fat gap was slightly blurred (fig. 1, the food mass completely blocked the intestine, Stomach and proximal small intestine
