【摘要】：背景與目的:腸易激綜合征(Irritable bowel syndrome,IBS)是一種多因素影響的功能性疾病,其病因和發(fā)病機(jī)制尚不明確。過(guò)敏性疾病與IBS可能存在關(guān)聯(lián),但研究較少。本研究目的是探討過(guò)敏性疾病對(duì)IBS患病率及其臨床表現(xiàn)的影響。方法:選取浙江大學(xué)醫(yī)學(xué)院附屬第二醫(yī)院2016.11～2017.03至過(guò)敏科門診就診的患者共108例作為過(guò)敏組,其中過(guò)敏性鼻炎(AR)患者49例作為AR亞組,蕁麻疹(CU)59例作為CU亞組。隨機(jī)選取本院病房陪護(hù)71例作為對(duì)照組。比較病例與對(duì)照的IBS檢出率及其相關(guān)胃腸道癥狀之間的差異,并分析其影響因素。結(jié)果:CU組IBS檢出率較一般人群有升高趨勢(shì)(OR=4.644,P=0.078),糞便稀爛程度(P=0.026)和糞便黏滯情況(P=0.015)也更為明顯。AR組嗜酸粒細(xì)胞(P=0.005)、總免疫球蛋白E(IgE)(P=0.030)較對(duì)照組顯著增高,戶塵螨IgE陽(yáng)性率較CU組顯著升高(P0.001)。過(guò)敏組中總IgE與腹脹的發(fā)生(OR=3.379,P=0.030)和腹脹嚴(yán)重程度(P=0.036)有關(guān)。戶塵螨IgE與腹脹的發(fā)生有關(guān)(OR=3.640,P=0.016),且為腹脹的獨(dú)立危險(xiǎn)因素。結(jié)論:CU患者有并發(fā)IBS的趨勢(shì),且以腹瀉為主。過(guò)敏性疾病患者總IgE和戶塵螨IgE與腹脹的發(fā)生和嚴(yán)重程度有關(guān),提示過(guò)敏性疾病對(duì)胃腸道癥狀的作用機(jī)制可能涉及由IgE介導(dǎo)的免疫反應(yīng)。戶塵螨IgE是腹脹的獨(dú)立危險(xiǎn)因素,提示該過(guò)敏原與胃腸道癥狀的密切聯(lián)系。
[Abstract]:Background & objective: Irritable bowel syndrome (IBSs) is a multifactorial functional disease, the etiology and pathogenesis of which are unclear.Allergic diseases may be associated with IBS, but less research has been done.The aim of this study was to investigate the effects of allergic diseases on the prevalence and clinical manifestations of IBS.Methods: 108 patients from the second affiliated Hospital of Zhejiang University Medical College were selected as allergic group, 49 patients with allergic rhinitis as AR subgroup and 59 patients with urticaria as CU subgroup.A total of 71 patients were randomly selected as control group.To compare the detection rate of IBS and the difference of gastrointestinal symptoms between cases and controls, and analyze the influencing factors.Results the detection rate of IBS in the 10% CU group was significantly higher than that in the general population (P 0.026) and the viscosity of feces (P 0.026). The positive rate of IBS in the AR group was significantly higher than that in the control group (P 0.005), and the total immunoglobulin IgE (0.030) in the AR group was significantly higher than that in the control group.The positive rate of IgE in dust mites was significantly higher than that in CU group (P 0.001).In allergy group, the total IgE was related to the occurrence of abdominal distention (OR3.379 P0.030) and the severity of abdominal distension (P0.036).IgE was related to the occurrence of abdominal distention and was an independent risk factor of abdominal distension.Conclusion there is a tendency of IBS in patients with% CU, and diarrhea is the main disease.The total IgE and IgE were related to the occurrence and severity of abdominal distension in patients with allergic diseases, suggesting that the mechanism of allergic diseases on gastrointestinal symptoms may be related to the immune response mediated by IgE.IgE is an independent risk factor for abdominal distension, suggesting that the allergen is closely associated with gastrointestinal symptoms.
