【摘要】：目的探討不同劑量生長(zhǎng)抑素治療肝硬化合并食管胃底靜脈曲張破裂出血的臨床效果。方法選取2013—2015年入住海南省人民醫(yī)院急診ICU及消化內(nèi)科的符合納入標(biāo)準(zhǔn)的102例肝硬化合并食管胃底靜脈曲張破裂出血患者,根據(jù)隨機(jī)數(shù)字表法將患者分為標(biāo)準(zhǔn)劑量組(A組)和加倍劑量組(B組),各51例。依據(jù)患者就診時(shí)既往出血次數(shù)(是否2次)對(duì)A、B兩組依次再分為A1組(既往出血2次)、A2組(既往出血≥2次),B1組(既往出血2次)、B2組(既往出血≥2次)。A組給予生長(zhǎng)抑素首劑4.1 ml靜脈注射后,再給予生長(zhǎng)抑素4.1 ml/h持續(xù)泵入,持續(xù)5 d。B組給予生長(zhǎng)抑素首劑8.2 ml靜脈注射后,再給予生長(zhǎng)抑素8.2 ml/h持續(xù)泵入,持續(xù)5 d。比較A組和B組、A1組和B1組、A2組和B2組的首次止血成功率、再出血率、治療有效率、病死率。結(jié)果 B組的首次止血成功率、治療有效率高于A組,再出血率低于A組(P0.05)。B2組的首次止血成功率高于A2組,再出血率低于A2組(P0.05)。結(jié)論對(duì)于肝硬化合并食管胃底靜脈曲張破裂出血患者,加倍劑量生長(zhǎng)抑素治療是有效的,尤其是對(duì)于既往出血≥2次的患者,可以明顯降低再出血率。
[Abstract]:Objective to investigate the clinical effect of different doses of somatostatin in the treatment of cirrhosis with esophageal varices bleeding.Methods 102 patients with cirrhosis complicated with esophageal and gastric variceal bleeding who were admitted to Hainan Provincial people's Hospital from 2013 to 2015 in emergency department of ICU and Department of Digestive Medicine were selected.Patients were randomly divided into two groups: standard dose group (n = 51) and double dose group (n = 51).According to the number of previous bleeding (2 times or not), the two groups were subdivided into group A1 (2 times of previous hemorrhage) and group A 2 (group B 1 with previous hemorrhage 鈮，
