【摘要】：目的分析細菌性肝膿腫患者超聲造影特點,以期提高診斷符合率。方法選擇2009年1月-2013年12月經(jīng)超聲造影檢查的30例細菌性肝膿腫患者41個病灶資料,回顧性分析細菌性肝膿腫常規(guī)超聲與超聲造影表現(xiàn),比較兩種檢查方法的診斷符合率。結(jié)果 30例細菌性肝膿腫患者中有21例單發(fā)、9例多發(fā),累及部位肝右葉18例、肝左葉9例、肝左葉均累及3例,膿腫大小為直徑2~9cm,平均(5.18±2.76)cm;超聲造影檢查:41個病灶均表現(xiàn)病灶縮小征,其中17個病灶呈花瓣樣強化、14個病灶呈環(huán)狀強化、10個病灶呈蜂窩狀強化;24例患者28個病灶呈典型的陡直快進快出曲線;超聲造影診斷符合率為93.33%,明顯高于常規(guī)超聲的73.33%,差異有統(tǒng)計學意義(χ2=9.24,P0.05)。結(jié)論超聲造影檢查集中了灰階成像、彩色多普勒血流頻譜和強化時間-強度曲線分析的優(yōu)勢,對提高細菌性肝膿腫超聲診斷符合率有較高的應(yīng)用價值。
[Abstract]:Objective to analyze the characteristics of contrast-enhanced ultrasonography in patients with bacterial liver abscess and to improve the diagnostic accuracy. Methods 41 lesions of 30 patients with bacterial liver abscess from January 2009 to 2013 were selected. The diagnostic accuracy of conventional ultrasound and contrast-enhanced ultrasonography for bacterial liver abscess was analyzed retrospectively. Results among the 30 patients with bacterial liver abscess, 9 had multiple lesions and 18 involved the right lobe of the liver. There were 9 cases of Zuo Ye and 3 cases of hepatic left lobe involvement. The size of abscess was 2 ~ 9 cm in diameter with an average of 5.18 鹵2.76 cm. Among them, 17 lesions were petal-like enhancement, 14 lesions were annular enhancement, and 10 lesions were honeycombed enhancement. In 24 cases, 28 lesions showed a typical steepness fast in and out curve. The diagnostic coincidence rate of contrast-enhanced ultrasonography was 93.33, which was significantly higher than that of conventional ultrasound (73.33). The difference was statistically significant (蠂 ~ 2 / 9.24) P 0.05.Conclusion Ultrasonography focuses on the advantages of gray scale imaging, color Doppler blood flow spectrum and enhanced time-intensity curve analysis. It is valuable to improve the diagnostic accuracy of ultrasonic diagnosis of bacterial liver abscess.
