【摘要】：目的：對特利加壓素預(yù)防腹腔穿刺術(shù)后循環(huán)功能障礙（post-paracentesis circulatory dysfunction，PPCD）的效果進行系統(tǒng)評價。 方法：通過PubMed、EMbase、Cochrane Library和中國知網(wǎng)等數(shù)據(jù)庫，檢索2013年10月前發(fā)表的有關(guān)肝硬化合并大量腹水患者在腹腔穿刺大量放液（large-volume paracentesis，LVP）后使用并比較特利加壓素與白蛋白預(yù)防PPCD效果的隨機對照試驗文獻，并采用RevMan5.2.5軟件進行Meta分析。 結(jié)果：特利加壓素組與白蛋白組相比，患者在LVP后平均動脈壓、血漿醛固酮濃度、血清肌酐濃度、血清鈉濃度及體重的變化無統(tǒng)計學(xué)差異。其加權(quán)均數(shù)差值（weighted mean difference，WMD）及95%置信區(qū)間（confidence interval，CI）分別為0.22（-2.02，2.45，，P=0.85）、-2.41（-188.26，183.44，P＝0.98）、2.02（-5.40，9.44，P＝0.59）、0.12（-1.59，1.82，P＝0.89）、-0.54（-1.41，0.33，P=0.23）。 結(jié)論：特利加壓素與白蛋白對預(yù)防PPCD的治療效果相近，可作為替代白蛋白治療的新選擇。
[Abstract]:Objective: to evaluate the effect of trivasopressin on preventing post-paracentesis circulatory function PPCDs after abdominal puncture. Methods: the database of PubMedus EMbase Cochrane Library and China KnowledgeNet were used. To retrieve the literature published before October 2013 on the use and comparison of trivasopressin and albumin in the prevention of PPCD in cirrhotic patients with massive ascites after massive peritoneal puncture. Meta analysis was performed with RevMan5.2.5 software. Results: the mean arterial pressure, plasma aldosterone concentration, serum creatinine concentration and serum creatinine concentration in patients with LVP were compared with those in albumin group. There was no statistical difference in serum sodium concentration and body weight. The weighted mean value of mean difference and confidence interval (95% confidence interval) were 0.22 ~ 2. 02% -2. 45% -2. 41 ~ 18. 26 ~ 183.44P0. 982.02- 5. 409.4 P0. 59 ~ 0. 59 and 0. 89 ~ 0. 54 ~ 0. 54 ~ 1. 41 ~ 1. 31 ~ 0. 33P0. 23 respectively. Conclusion: the therapeutic effect of trivasopressin and albumin on the prevention of PPCD is similar, and it can be used as a new alternative to albumin therapy.
