【摘要】：目的研究肝硬化后門靜脈高壓病例大血管血管壁的組織結(jié)構(gòu)改變,探討誘導(dǎo)型一氧化氮合酶(iNOS)在肝硬化發(fā)生進(jìn)程中的作用。方法收集肝硬化肝移植病例的門靜脈左支、左肝動(dòng)脈和肝左靜脈27例作為病例組,同期外傷性肝切除相應(yīng)血管15例為對(duì)照組。分別予以HE染色、Masson染色,肝動(dòng)脈加用彈力纖維染色和兔抗人iNOS多克隆抗體免疫組織化學(xué)。結(jié)果病例組門靜脈內(nèi)膜、中膜增厚,中膜平滑肌細(xì)胞增生、肥大,部分突出至內(nèi)膜下;肝動(dòng)脈內(nèi)膜不規(guī)則增生,動(dòng)脈管腔變窄,動(dòng)脈壁內(nèi)彈力膜變平、甚至斷裂,中膜平滑肌向內(nèi)膜增生;肝靜脈壁組織結(jié)構(gòu)沒(méi)有明顯改變。iNOS在肝動(dòng)脈平滑肌細(xì)胞胞質(zhì)呈高表達(dá)。結(jié)論肝硬化患者肝動(dòng)脈與門靜脈血管壁會(huì)繼發(fā)組織結(jié)構(gòu)重建,這是人體自身代償機(jī)制的結(jié)果,iNOS的高表達(dá)加速了該組織結(jié)構(gòu)重建的發(fā)生、發(fā)展。
[Abstract]:Objective to study the changes of tissue structure of large vascular wall in patients with portal hypertension after cirrhosis. To investigate the role of inducible nitric oxide synthase (iNOS) in the progression of liver cirrhosis. Methods the left portal vein, left hepatic artery and left hepatic vein were collected from 27 patients with liver transplantation. At the same time, 15 cases of the corresponding blood vessels were treated with HE staining and Masson staining. Results in the case group, the portal vein intima was thickened, the medial smooth muscle cells proliferated and hypertrophic, and some of them protruded to the subintima. The intima of the hepatic artery was irregular hyperplasia, the arterial lumen became narrow, the elastic membrane in the wall of the artery flattened or even broke, and the smooth muscle of the middle membrane proliferated to the intima. There is no obvious change in the structure of hepatic vein wall. INOS is highly expressed in the cytoplasm of smooth muscle cells of hepatic artery. Conclusion the hepatic artery and portal vein wall of liver cirrhosis patients will be secondary to tissue structure reconstruction. This is the result of the compensatory mechanism of human body. The high expression of iNOS accelerates the reconstruction of the tissue structure.
