【摘要】：目的了解住院肝病患者肺部感染肺炎克雷伯菌及其耐藥情況,為臨床合理應(yīng)用抗菌藥物提供依據(jù)。方法通過細(xì)菌分離鑒定和藥敏試驗(yàn)方法,對山東某傳染病�？漆t(yī)院住院肝病患者送檢痰標(biāo)本進(jìn)行病原菌檢測和分析。結(jié)果從65例住院肝病并發(fā)肺部感染患者痰標(biāo)本中分離出65株肺炎克雷伯菌,其中檢出產(chǎn)超廣譜β-內(nèi)酰胺酶菌株(ESBLs)21株,占32.31%。從痰液分離的肺炎克雷伯菌對18種臨床常用抗菌藥物均不同程度耐藥,產(chǎn)ESBLs菌株耐藥率明顯高于非ESBLs菌株。臨床分離的肺炎克雷伯菌僅對亞胺培南/西司他丁和美羅培南等碳青霉烯類比較敏感。結(jié)論住院肝病患者肺部感染的肺炎克雷伯菌中產(chǎn)ESBLs菌株比例較高,耐藥嚴(yán)重,僅對碳青霉烯類抗菌藥物敏感,提示應(yīng)根據(jù)藥敏試驗(yàn)結(jié)果選用抗菌藥物。
[Abstract]:Objective to understand the situation of Klebsiella pneumoniae and its drug resistance in hospitalized patients with liver disease, and to provide evidence for the rational use of antimicrobial agents in clinic. Results 65 strains of Klebsiella pneumoniae were isolated from the sputum samples of 65 hospitalized patients with pulmonary infection. Among them, 21 strains (32.31%) produced extended-spectrum 尾 -lactamases (ESBLsN). Klebsiella pneumoniae isolated from sputum were resistant to 18 kinds of commonly used antibiotics in different degrees. The drug resistance rate of ESBLs producing strains was significantly higher than that of non-#en1# strains. Klebsiella pneumoniae isolated clinically was only sensitive to carbapenem such as imipenem / cilastatin and meropenem. Conclusion inpatients with liver diseases are more sensitive to carbapenem. The proportion of ESBLs producing strains of Klebsiella pneumoniae in pulmonary infection was higher. The drug resistance is serious and only sensitive to carbapenes, suggesting that antimicrobial agents should be selected according to the results of drug sensitivity test.
