肝移植術(shù)后頑固性脾功能亢進(jìn)預(yù)測(cè)因素的分析

目的1.評(píng)估原位肝移植術(shù)對(duì)肝硬化門靜脈高壓后脾功能亢進(jìn)的緩解效果;2.分析肝移植術(shù)后頑固性脾功能亢進(jìn)的預(yù)測(cè)因素并評(píng)價(jià)其預(yù)測(cè)價(jià)值,為術(shù)前識(shí)別可能發(fā)生頑固性脾亢的高危病人提供依據(jù),從而指導(dǎo)圍術(shù)期診療方案,以期降低頑固性脾亢的發(fā)生率。方法回顧性分析55例符合納入排除標(biāo)準(zhǔn)的終末期肝病伴脾功能亢進(jìn)行原位肝移植術(shù)病人的臨床資料,通過觀察其圍手術(shù)期1年內(nèi)脾體積、脾厚度、門靜脈流速、血小板、白細(xì)胞計(jì)數(shù)的變化,評(píng)估原位肝移植對(duì)脾功能亢進(jìn)的緩解效果;根據(jù)觀察終點(diǎn)的化驗(yàn)檢查結(jié)果,將病人分為脾功能亢進(jìn)恢復(fù)組和頑固性脾功能亢進(jìn)組,分析肝移植術(shù)后頑固性脾功能亢進(jìn)的預(yù)測(cè)因素,計(jì)算其靈敏度、特異度、約登指數(shù)、最佳臨界值、曲線下面積、陽性似然比、陰性似然比,評(píng)價(jià)其對(duì)頑固性脾功能亢進(jìn)預(yù)測(cè)的價(jià)值。結(jié)果(1)術(shù)后脾體積(562.90±49.16cm3)較術(shù)前(850.50±77.99cm3)顯著下降(P<0.05),下降比例為(31.70±2.76)%;(2)術(shù)后不同時(shí)間點(diǎn)的脾厚度均較術(shù)前顯著下降(P<0.05),于術(shù)后1個(gè)月開始趨于穩(wěn)定;(3)術(shù)后不同時(shí)間點(diǎn)的門靜脈流...
