【摘要】：目的探討應用示指尺側(cè)指固有神經(jīng)及其背側(cè)支移位修復拇指兩側(cè)指固有神經(jīng)撕脫離斷傷的臨床療效。方法 2007年1月—2015年5月,采用示指尺側(cè)指固有神經(jīng)及其背側(cè)支移位修復23例拇指兩側(cè)指固有神經(jīng)撕脫離斷傷患者。其中男17例,女6例;年齡16~63歲,平均32歲。致傷原因:機器絞傷10例,電鋸傷8例,堅銳物品剮傷5例。拇指完全撕脫離斷8例,拇指不完全離斷2例,拇指掌側(cè)裂傷13例(合并屈肌腱斷裂7例)。受傷至手術(shù)時間為1.0~3.5 h,平均2.2 h。結(jié)果術(shù)后患者切口均Ⅰ期愈合,10例再植拇指全部成活。23例患者均獲隨訪,隨訪時間5個月~2年8個月,平均1年4個月�；贾钢父垢杏X恢復良好,末次隨訪時兩點辨別覺為3~9 mm,平均6.8 mm。按照中華醫(yī)學會手外科學會上肢部分功能評定試用標準評價,拇指指腹感覺達S418例、S3+5例,示指尺側(cè)感覺為S3。結(jié)論應用示指尺側(cè)指固有神經(jīng)及其背側(cè)支移位修復拇指指固有神經(jīng)撕脫離斷傷,能恢復拇指指腹良好感覺,手術(shù)操作簡便,療效確切。
[Abstract]:Objective to investigate the clinical effect of the transposition of the ulna digital nerve and its dorsal branch in the repair of avulsion injury of the digital nerve on both sides of the thumb. Methods from January 2007 to May 2015, 23 patients with bilateral finger avulsion injury were repaired by transposition of ulna digital nerve and its dorsal branch. There were 17 males and 6 females with an average age of 32 years. The causes of injury were machine strangulation in 10 cases, chainsaw injury in 8 cases and sharp object wound in 5 cases. There were 8 cases of complete avulsion of thumb, 2 cases of incomplete amputation of thumb and 13 cases of palm laceration of thumb (7 cases complicated with rupture of flexor tendon). The time from injury to operation was 1.0 脳 3.5 h, with an average of 2.2 h. Results all the incisions healed in the first stage after operation, and all the 10 cases survived. All 23 cases were followed up for 5 months to 2 years and 8 months, with an average of 1 year and 4 months. The finger abdomen feeling recovered well, and the two point discrimination sense was 3 鈮，
