手術(shù)部位感染與手衛(wèi)生相關(guān)性研究

【摘要】:目的探討外科手術(shù)部位感染的發(fā)生率、常見(jiàn)致病菌譜及其耐藥性,并分析手術(shù)部位感染發(fā)生率與手衛(wèi)生產(chǎn)品消耗量之間的統(tǒng)計(jì)學(xué)相關(guān)性。方法回顧性分析2015年南京鼓樓醫(yī)院與南京軍區(qū)南京總醫(yī)院診治的外科手術(shù)病人資料,遴選出發(fā)生手術(shù)部位感染的研究人群,分析并總結(jié)其手術(shù)部位感染相關(guān)資料,并針對(duì)各科室手術(shù)部位感染發(fā)生率與該科室手衛(wèi)生產(chǎn)品消耗量進(jìn)行相關(guān)性分析及多元回歸分析。結(jié)果在30 678例外科手術(shù)中,共307例發(fā)生手術(shù)部位感染,總體發(fā)生率為1.00%;不同科室的手術(shù)部位感染發(fā)生率存在較大差異,以普通外科最高(2.14%)、神經(jīng)外科最低(0.14%);手術(shù)部位感染常見(jiàn)致病菌包括大腸埃希菌(35.8%)、金黃色葡萄球菌(13.20%)、糞腸球菌(7.84%)、銅綠假單胞菌(5.89%)、肺炎克雷伯菌(4.90%)等。相應(yīng)病原菌的耐藥率存在較大差異。外科各科室的手術(shù)部位感染發(fā)生率與三種手衛(wèi)生產(chǎn)品消耗量均呈顯著負(fù)相關(guān)(r=-0.83~-0.88、P=0.0031~0.0083);多元回歸分析進(jìn)一步顯示:免洗手消毒液使用量顯著影響手術(shù)部位感染發(fā)生率(P=0.023)。結(jié)論手衛(wèi)生是影響手術(shù)部位感染發(fā)生率的關(guān)鍵因素,推動(dòng)醫(yī)務(wù)人員手衛(wèi)生依從性或有助于減少手術(shù)部位感染。了解手術(shù)部位感染臨床特征有助于制定個(gè)體化、區(qū)域性手術(shù)部位感染防治策略。
[Abstract]:Objective to investigate the incidence of surgical site infection, common pathogenic bacteria spectrum and drug resistance, and to analyze the statistical correlation between the incidence of surgical site infection and the consumption of hand hygiene products. Methods the data of surgical patients diagnosed and treated in Nanjing Gulou Hospital and Nanjing General Hospital of Nanjing military region in 2015 were analyzed retrospectively. the research population with surgical site infection was selected, the related data of surgical site infection were analyzed and summarized, and the correlation analysis and multiple regression analysis were carried out between the incidence of surgical site infection and the consumption of hand hygiene products in each department. Results in 30678 cases, 307 cases had surgical site infection, the overall incidence was 1.00%, the incidence of surgical site infection in different departments was the highest (2.14%) and neurosurgery was the lowest (0.14%), and the incidence of surgical site infection was the highest in different departments (2.14%) and the lowest in neurosurgery (0.14%). The common pathogens of surgical site infection included Escherichia coli (35.8%), Staphylococcus aureus (13.20%), Enterococcus faecalis (7.84%), Pseudomonas aeruginosa (5.89%) and Klebsiella pneumoniae (4.90%). There are great differences in the drug resistance rate of the corresponding pathogens.
