ConView腦電意識(shí)狀態(tài)法與BIS法監(jiān)測(cè)患者異丙酚鎮(zhèn)靜深度準(zhǔn)確性的比較

【摘要】:目的:通過(guò)與BIS法比較,評(píng)價(jià)ConView腦電意識(shí)狀態(tài)法監(jiān)測(cè)患者異丙酚鎮(zhèn)靜深度的準(zhǔn)確性。方法:全麻下行擇期手術(shù)患者30例,性別不限,年齡41~64歲,體重指數(shù)18~28kg/m2,ASA分級(jí)Ⅰ或Ⅱ級(jí)。常規(guī)術(shù)前準(zhǔn)備。入室后建立肘正中靜脈通路,輸注復(fù)方氯化鈉注射液,常規(guī)監(jiān)測(cè)ECG、HR、SpO2和MAP。按照要求放置BIS與ConView腦電意識(shí)狀態(tài)監(jiān)測(cè)儀專(zhuān)用傳感器電極片,確保各電極皮膚電阻小于5.0kΩ,信號(hào)質(zhì)量指數(shù)(Signal Quality Index,SQI)大于90。采用Marsh藥代動(dòng)力學(xué)參數(shù)TCI輸注異丙酚,設(shè)定初始Ce為1.2μg/ml,待Ce穩(wěn)定后以0.3μg/ml為梯度遞增,并同時(shí)行OAA/S評(píng)分。記錄開(kāi)始輸注異丙酚前進(jìn)行OAA/S評(píng)分及TCI異丙酚期間OAA/S評(píng)分依次為4分、3分、2分、1分時(shí)的BIS值、AI值和異丙酚Ce。維持生命體征指標(biāo)在正常范圍內(nèi)。對(duì)BIS值、AI值及Ce分別與OAA/S評(píng)分行Spearman等級(jí)相關(guān)分析,對(duì)BIS值、AI值分別與Ce行Pearson相關(guān)分析,相關(guān)系數(shù)差異的顯著性檢驗(yàn)采用u檢驗(yàn),以P0.05為差異有統(tǒng)計(jì)學(xué)意義。結(jié)果:BIS值、AI值、Ce與OAA/S評(píng)分的相關(guān)系數(shù)分別為rBIS-OAA/S=0.962、rAI-OAA/S=0.960、rCe-OAA/S=㧟0.795(P0.05);BIS值、AI值與Ce的相關(guān)系數(shù)分別為rBIS-Ce=㧟0.736、rAI-Ce=㧟0.737(P0.05)。rBIS-OAA/S及rAI-OAA/S之間、rBIS-Ce及rAI-Ce之間比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:成年患者非傷害性刺激條件下,ConView腦電意識(shí)狀態(tài)法監(jiān)測(cè)患者異丙酚鎮(zhèn)靜深度準(zhǔn)確,且與BIS法相比無(wú)明顯差異。
