【摘要】：目的:探討植骨后外固定支架固定聯(lián)合健骨促愈中藥對(duì)脛骨骨折不愈合關(guān)節(jié)功能改善、骨痂評(píng)分及愈合時(shí)間的影響。方法:研究對(duì)象選取醫(yī)院2013年4月—2015年7月收治脛骨骨折不愈合患者共130例,以隨機(jī)數(shù)字表法分為對(duì)照組(65例)和觀察組(65例),分別采用植骨后外固定支架固定單用和在此基礎(chǔ)上加用健骨促愈中藥輔助治療;比較兩組患者近期療效,骨折愈合時(shí)間,治療后骨痂評(píng)分,治療前后Lysholm評(píng)分,HSS評(píng)分及實(shí)驗(yàn)室指標(biāo)水平等。結(jié)果:觀察組患者近期療效顯著優(yōu)于對(duì)照組(P0.05);觀察組患者骨折愈合時(shí)間顯著短于對(duì)照組(P0.05);觀察組患者治療后Lysholm評(píng)分和HSS評(píng)分均顯著高于對(duì)照組、治療前(P0.05);觀察組患者治療后骨痂評(píng)分顯著高于對(duì)照組(P0.05);同時(shí)觀察組患者治療后實(shí)驗(yàn)室指標(biāo)水平均顯著高于對(duì)照組、治療前(P0.05)。結(jié)論:植骨后外固定支架固定聯(lián)合健骨促愈中藥治療脛骨骨折不愈合可有效縮短骨折愈合時(shí)間,改善膝關(guān)節(jié)功能,并有助于上調(diào)BMP-2,IGF-1,IGF-2及VEGF水平。
[Abstract]:Objective: to investigate the effect of external fixation after bone grafting combined with traditional Chinese medicine for promoting bone healing on the improvement of joint function, eschar score and healing time of tibia fracture. Methods: from April 2013 to July 2015, 130 patients with nonunion of tibia fracture were randomly divided into control group (65 cases) and observation group (65 cases). The short-term curative effect, fracture healing time, eschar score, Lysholm score, HSS score and laboratory index level before and after treatment were compared between the two groups. Results: the short-term curative effect of the observation group was significantly better than that of the control group (P 0.05), the fracture healing time of the observation group was significantly shorter than that of the control group (P 0.05), the Lysholm score and HSS score of the observation group were significantly higher than those of the control group after treatment (P 0.05), and the eschar score of the observation group was significantly higher than that of the control group after treatment (P 0.05). At the same time, the level of laboratory indexes in the observation group was significantly higher than that in the control group after treatment (P 0.05). Conclusion: the treatment of nonunion of tibia fracture with external fixation after bone grafting combined with traditional Chinese medicine can effectively shorten the time of fracture healing, improve the function of knee joint, and help to up-regulate the levels of BMP-2,IGF-1,IGF-2 and VEGF.
