【摘要】：目的:評(píng)估在全麻手術(shù)期間應(yīng)用T-Line300監(jiān)測(cè)連續(xù)無(wú)創(chuàng)動(dòng)脈血壓與有創(chuàng)動(dòng)脈血壓的一致性。方法:術(shù)中需行有創(chuàng)血壓監(jiān)測(cè)的擇期全麻手術(shù)患者29例,每間隔15min同步記錄一組由T-line 300測(cè)得的無(wú)創(chuàng)動(dòng)脈血壓和由橈動(dòng)脈或足背動(dòng)脈置管測(cè)得的有創(chuàng)血壓。術(shù)中給予去氧腎上腺素或酚妥拉明治療低血壓或高血壓的患者,給藥前即刻及給藥后10min內(nèi),每間隔1min同步記錄一組無(wú)創(chuàng)血壓和有創(chuàng)血壓值。用Bland-Altman分析評(píng)價(jià)兩種測(cè)量方法結(jié)果的一致性,四象限散點(diǎn)圖觀察T-Line300監(jiān)測(cè)血壓變化趨勢(shì)的能力。結(jié)果:27位患者,1381對(duì)無(wú)創(chuàng)動(dòng)脈血壓和有創(chuàng)血壓(SAP、DAP各465對(duì),MAP 451對(duì))進(jìn)入最后的統(tǒng)計(jì)學(xué)分析。SAP、DAP、MAP的平均誤差(bias±sd)分別為-5.8±14.5、4.2±11.4、2.1±10.1 mm Hg。應(yīng)用血管活性藥物后,連續(xù)無(wú)創(chuàng)血壓的變化值與有創(chuàng)血壓變化值之間的相關(guān)系數(shù)分別為△SAP:0.5479、△DAP:0.5805、△MAP:0.5277。結(jié)論:T-Line300用于全麻手術(shù)病人能提供較準(zhǔn)確的SAP、DAP、MAP測(cè)量,但其精確性和在血壓快速變化時(shí)的反應(yīng)性還有待提高。
[Abstract]:Objective: to evaluate the consistency of continuous noninvasive arterial blood pressure and invasive arterial blood pressure monitored by T-Line300 during general anesthesia. Methods: 29 patients undergoing elective general anesthesia with invasive blood pressure monitoring were enrolled in this study. One group of noninvasive arterial blood pressure measured by T-line 300 and invasive blood pressure measured by radial artery or dorsalis pedis artery catheterization were recorded synchronously in each septum 15min. Patients with hypotension or hypertension were treated with norepinephrine or phentolamine during operation. A group of noninvasive blood pressure and invasive blood pressure were recorded immediately before and after administration of 10min. Bland-Altman analysis was used to evaluate the consistency of the results of the two methods, and the ability of T-Line300 to monitor the changing trend of blood pressure was observed by four-quadrant scatter plot. Results: 1381 pairs of noninvasive arterial blood pressure and 1381 pairs of invasive blood pressure (SAP,DAP 465 pairs, MAP 451 pairs) entered the final statistical analysis. The average error (bias 鹵SD) of SAP and DAP,MAP was-5.8 鹵14.5, 4.2 鹵11.4, 2.1 鹵10.1 mm Hg., respectively. After the application of vasoactive drugs, the correlation coefficients between the change of continuous noninvasive blood pressure and the change of invasive blood pressure were SAP:0.5479, DAP:0.5805, MAP:0.5277., respectively. Conclusion: T-Line300 can provide accurate SAP,DAP,MAP measurement in patients undergoing general anesthesia, but its accuracy and responsiveness to rapid change of blood pressure still need to be improved.
