【摘要】：背景:目前,髖關節(jié)置換和空心加壓螺釘內(nèi)固定是治療老年股骨頸骨折的主要方法。但老年患者由于骨質(zhì)疏松,內(nèi)固定往往失敗。目的:分析空心釘內(nèi)固定治療股骨頸骨折患者術后關節(jié)功能,并探索影響空心釘內(nèi)固定后髖關節(jié)功能的相關因素。方法:回顧性分析空心釘內(nèi)固定治療的股骨頸骨折患者并獲得完整2年隨訪的89例患者臨床資料,就性別、年齡、骨折類型(Garden分型)、骨量、體質(zhì)量指數(shù)、空心釘固定結構、復位指數(shù)、手術時間、內(nèi)固定后至患肢部分負重時間及2年隨訪時髖關節(jié)功能Harris評分等因素進行分析。應用SPSS 18.0統(tǒng)計學軟件進行等級相關分析、有序多分類logistic回歸分析。結果與結論:(1)標準正態(tài)Q-Q概率圖及正態(tài)性檢驗結果顯示,所有獲得隨訪的患者術后2年Harris評分呈偏態(tài)分布(P=0.000);(2)等級秩相關分析顯示:年齡、骨折類型、骨量、復位指數(shù)與髖關節(jié)功能評分呈相關性(P0.05或P0.01);(3)有序多分類Logistic回歸分析結果顯示:年齡、骨折類型、骨量、復位指數(shù)4個自變量對髖關節(jié)功能評分的影響具有顯著性意義;(4)結果說明,年齡、骨折類型、骨量、復位指數(shù)是空心釘內(nèi)固定治療股骨頸骨折術后髖關節(jié)功能的重要影響因素。
[Abstract]:Background: hip replacement and hollow compression screw fixation are the main methods for the treatment of femoral neck fracture in the elderly.
