【摘要】：背景:目前,髖關(guān)節(jié)置換和空心加壓螺釘內(nèi)固定是治療老年股骨頸骨折的主要方法。但老年患者由于骨質(zhì)疏松,內(nèi)固定往往失敗。目的:分析空心釘內(nèi)固定治療股骨頸骨折患者術(shù)后關(guān)節(jié)功能,并探索影響空心釘內(nèi)固定后髖關(guān)節(jié)功能的相關(guān)因素。方法:回顧性分析空心釘內(nèi)固定治療的股骨頸骨折患者并獲得完整2年隨訪的89例患者臨床資料,就性別、年齡、骨折類型(Garden分型)、骨量、體質(zhì)量指數(shù)、空心釘固定結(jié)構(gòu)、復(fù)位指數(shù)、手術(shù)時(shí)間、內(nèi)固定后至患肢部分負(fù)重時(shí)間及2年隨訪時(shí)髖關(guān)節(jié)功能Harris評(píng)分等因素進(jìn)行分析。應(yīng)用SPSS 18.0統(tǒng)計(jì)學(xué)軟件進(jìn)行等級(jí)相關(guān)分析、有序多分類logistic回歸分析。結(jié)果與結(jié)論:(1)標(biāo)準(zhǔn)正態(tài)Q-Q概率圖及正態(tài)性檢驗(yàn)結(jié)果顯示,所有獲得隨訪的患者術(shù)后2年Harris評(píng)分呈偏態(tài)分布(P=0.000);(2)等級(jí)秩相關(guān)分析顯示:年齡、骨折類型、骨量、復(fù)位指數(shù)與髖關(guān)節(jié)功能評(píng)分呈相關(guān)性(P0.05或P0.01);(3)有序多分類Logistic回歸分析結(jié)果顯示:年齡、骨折類型、骨量、復(fù)位指數(shù)4個(gè)自變量對(duì)髖關(guān)節(jié)功能評(píng)分的影響具有顯著性意義;(4)結(jié)果說(shuō)明,年齡、骨折類型、骨量、復(fù)位指數(shù)是空心釘內(nèi)固定治療股骨頸骨折術(shù)后髖關(guān)節(jié)功能的重要影響因素。
[Abstract]:Background: hip replacement and hollow compression screw fixation are the main methods for the treatment of femoral neck fracture in the elderly.
