溫度敏感性瞬時受體通道M4在人類膝關節(jié)軟骨退變中的表達及意義

【摘要】:目的:檢測在人關節(jié)軟骨中是否表達溫度敏感性瞬時受體通道TRPM4,探討其在不同年齡正常關節(jié)軟骨及不同程度骨關節(jié)炎患者關節(jié)軟骨上的變化趨勢,研究TRPM4通道與關節(jié)軟骨退變的關系。方法:收集山西醫(yī)科大學第二醫(yī)院骨科外傷截肢患者正常膝關節(jié)軟骨,根據(jù)年齡分為20~29歲組,30~39歲組,40~49歲組,每組6例;收集OA患者膝關節(jié)置換關節(jié)軟骨,根據(jù)破壞程度分為軟骨破壞輕組和軟骨破壞重組,每組6例。番紅O染色結果評價不同年齡正常膝關節(jié)及骨關節(jié)炎患者軟骨退變程度,實時熒光定量聚合酶鏈反應法測定TRPM4 m RNA在基因水平的變化,免疫印跡法測定TRPM4蛋白水平的表達,免疫熒光和免疫組化分析TRPM4在關節(jié)軟骨及細胞表面的分布。結果:番紅O染色顯示,隨著患者年齡的增加,關節(jié)軟骨表面逐漸由光滑變毛糙,細胞間區(qū)染色逐漸由深紅色變?yōu)闇\紅色,OA患者糖胺多糖退變更明顯。人膝關節(jié)軟骨上存在TRPM4蛋白通道。隨著年齡的增加,正常軟骨中各組TRPM4m RNA和蛋白逐漸增加,差別有統(tǒng)計學意義(P0.05),并且年齡和TRPM4蛋白表達量呈正相關(r=0.632,P0.01)。隨著關節(jié)軟骨破壞的加重,軟骨中TRPM4的m RNA和蛋白顯著增加,差別有統(tǒng)計學意義(P0.05),且TRPM4蛋白表達量和破壞程度正相關(r=0.714,P0.01)。免疫熒光顯示,TRPM4主要位于軟骨細胞膜上。免疫組化顯示,TRPM4位于軟骨淺中層。結論:TRPM4在人類膝關節(jié)軟骨細胞中表達,且存在于關節(jié)軟骨的淺中層細胞的細胞膜上。在正常人群中,隨著年齡的增長,TRPM4的表達量逐漸增加,在骨關節(jié)炎患者中,隨著破壞的加重,TRPM4表達量逐漸增加,并且,TRPM4的表達量與年齡變化及骨關節(jié)炎軟骨破壞程度呈正相關。目前,TRPM4在軟骨細胞的生理作用和病理生理機制仍不清楚,下一步我們的研究重點將關注于TRPM4通道蛋白在正常軟骨細胞和關節(jié)炎軟骨細胞中的功能,以便關節(jié)軟骨細胞退變的保護和預防,為治療骨關節(jié)炎提供新的靶點。
